The paper presents the development of a method for assessing auditory perception and the effectiveness of applying hearing aids for hard-of-hearing people during short-term (up to 7 days) and longer-term (up to 3 months) use. The method consists of a survey based on the APHAB questionnaire. Additional criteria such as the degree of hearing loss, technological level of hearing aids used, as well as the user experience are taken into consideration. A web-based application is developed, allowing answering the survey questions from any computer with Internet access. The results of the benefit obtained from the use of hearing aids in various acoustic environments, taking into account the time of their use, are presented and compared to the earlier outcomes. The research results show that in the first period of use of hearing aids, speech perception improves, especially in noisy environments. The perception of unpleasant sounds also increases, which may lead to deterioration of hearing aid acceptance by their users.

The study presents evaluating the effectiveness of the hearing aid fitting process in the short-term use (7 days). The evaluation method consists of a survey based on the APHAB (Abbreviated Profile of Hearing Aid Benefit) questionnaire. Additional criteria such as a degree of hearing loss, number of hours and days of hearing aid use as well as the user’s experience were also taken into consideration. The outcomes of the benefit obtained from the hearing aid use in various listening environments for 109 hearing aid users are presented, including a degree of their hearing loss. The research study results show that it is possible to obtain relevant and reliable information helpful in assessing the effectiveness of the short-term (7 days) hearing aid use. The overall percentage of subjects gaining a benefit when communicating in noise is the highest of all the analyzed and the lowest in the environment with reverberation. The statistical analysis performed confirms that in the listening environments in which conversation is held, a subjective indicator determined by averaging benefits for listening situations individually is statistically significant with respect to the degree of hearing loss. Statistically significant differences depending on the degree of hearing loss are also found separately for noisy as well as reverberant environments. However, it should be remembered that this study is limited to three types of hearing loss, i.e. mild, moderate and severe. The acceptance of unpleasant sounds gets the lowest rating. It has also been observed that in the initial period of hearing aid use, the perception of unpleasant sounds has a big influence on the evaluation of hearing improvement.

Osoby niedosłyszące bardzo często borykają się z problemem nieodpowiednio dobranego aparatu słuchowego. Najczęstszym problemem wśród osób noszących pomoce słuchowe jest niewłaściwe rozumienie mowy w różnych warunkach akustycznych. Obecnie aparaty ustawiane są za pomocą specjalistycznych programów i urządzeń na podstawie subiektywnych odczuć pacjenta. Przedstawiona w niniejszym artykule metoda dopasowania aparatów słuchowych przy użyciu systemu SpeechPro i urządzenia Avant REM Speech+ pozwala na obiektywne dopasowanie aparatu słuchowego każdej osobie niedosłyszącej bez jej czynnego udziału. Wyniki i dyskusja: Urządzenie Avant REM Speech+ całkowicie spełnia swoje zadanie – dopasowuje aparat słuchowy do danego ubytku słuchu bez ingerencji protetyka słuchu oraz subiektywnych odczuć pacjenta. Ustalenie natężenia dźwięku na zewnątrz i wewnątrz ucha pacjenta pozwala protetykowi słuchu dobrać rzeczywiste wzmocnienie niezbędne do danego niedosłuchu. Urządzenie jest bardzo dokładne, zakres błędu wynosi ±/1 dB. Wykres audiogramu oraz charakterystyka dopasowania są zbliżone do siebie, co odpowiada bardzo dokładnemu uzyskaniu skutecznego dopasowania aparatu słuchowego.

Deafness is civilization disease, which affects, not only older people, but also young people and children. The most common problem among hearing aid users is improper speech understanding in various acoustic conditions. The method described in following article is about hearing aid fitting, using SpeechPro and Avant REM Speech+ device allowing objective fitting of hearing aids to any hearing impaired person without active participation. Results and discussion: The Avant REM Speech+ device completely fulfills its purpose – it fits hearing system to the specific hearing loss without intervention of the hearing care professional and the subjective feelings of the patient. The chart of audiogram and fit characteristics are close to each other, which responds to very accurate attainment of effective fit of the hearing instrument.

Słuch jest zmysłem pozwalającym na odbieranie fal dźwiękowych. Narządem słuchowym są uszy. Coraz częściej człowiek spotyka się z ubytkiem słuchu. Historia zastosowania pierwszych urządzeń wspomagających słyszenie sięga aż starożytności. Zaczęto używać metalowe tuby. Wynalezienie mikrofonu, wzmacniaczy i tranzystorów spowodowało szybki postęp w rozwoju i miniaturyzacji aparatów słuchowych. Dzisiejsze aparaty słuchowe są dostosowywane do każdego pacjenta indywidualnie. Możemy wyróżnić aparaty słuchowe zauszne typu BTE i typu RITE, a także aparaty wewnątrzuszne ITE i wewnątrzkanałowe CIC.

This paper presents a literature review based on the generation of hearing aids, with particular emphasis on the anatomical structures of the ear and hearing loss affecting the structure and type of prostheses. An important aspect is the analysis of future solutions, taking into account the advantages and disadvantages of current hearing aids and their uses.

Content available remote Development of cartilage conduction hearing aid

Purpose: The potential demand for hearing aids is increasing in accordance with aging of populations in many developed countries. Because certain patients cannot use air conduction hearing aids, they usually use bone conduction hearing aids. However, bone does not transmit sound as efficiently as air, and bone conduction hearing aids require surgery (bone anchored hearing aid) or great pressure to the skull. The first purpose of this study is to examine the efficacy of a new sound conduction pathway via the cartilage. The second purpose is to develop a hearing aid with a cartilage conduction transducer for patients who cannot use regular air conduction hearing aids. Design/methodology/approach: We examined the hearing ability of a patient with atresia of both external auditory meatuses via three kinds of conduction pathways (air, bone, and cartilage). After the best position for the cartilage conduction transducer was found, audiometric evaluation was performed for his left ear with an insertion earphone (air conduction), a bone conduction transducer, and a cartilage conduction transducer. Then we made a new hearing aid using cartilage conduction and got subjective data from the patients. Findings: The tragal cartilage was the best position for the cartilage conduction transducer. The patient’s mean hearing levels were 58.3 dBHL, 6.7 dBHL, and 3.3 dBHL for air conduction, bone conduction, and cartilage conduction respectively. The hearing ability of the patients obtained from the cartilage conduction hearing aid was comparable to those from the bone conduction hearing aid. Practical implications: Hearing levels using cartilage conduction are very similar to those via bone conduction. Cartilage conduction hearing aids may overcome the practical disadvantages of bone conduction hearing aids such as pain and the need for surgery. Originality/value: We have clarified the efficacy of the cartilage conduction pathway and developed a prototype ‘cartilage conduction hearing aid’, which is the first hearing aid to use sound transmission via cartilage.
