Pulmonary lymphangitic carcinomatosis is a special type of diffuse metastasis of carcinoma in the lymphatic vessels of the lungs. Lymphangitic carcinomatosis is commonly observed in malignancies of the breast, lung, pancreas, colon and cervix as a strong marker for poor prognosis. Presenting with common respiratory symptoms, it may be easily misdiagnosed as other pulmonary interstitial diseases. Pulmonary lymphangitic carcinomatosis is a rare manifestation of metastatic gastric cancer. The presented case describes a patient with pulmonary lymphangitic carcinomatosis in the course of stomach cancer. The primary diagnosis was established based on the exclusion of other interstitial lung diseases, with the use of imaging techniques as well as biochemical, microbiological and cytological findings. The patient’s general condition was very severe, rendering him bedridden and therefore not eligible for any treatment, including chemotherapy. He died suddenly before final verification of the diagnosis. Pulmonary lymphangitic carcinomatosis should be suspected in patients with advanced gastric cancer, presenting with symptoms and signs of respiratory disease. Imaging techniques are mostly helpful to establish the diagnosis.

Both the adrenergic and the cholinergic components of the autonomic nervous system (ANS) have been found to be an important source of nerve fibres supplying the lungs. On the other hand, data regarding the innervation of the pulmonary tissue in breeding animals are surprisingly scarce. Therefore, in the present study noradrenergic and acetylcholinesterase-positive (AChE-positive) innervation of the lungs of sexually immature pigs was studied using histochemical methods. Studies were performed on six juvenile female pigs (aged 9 weeks, body weight 15–20 kg). Samples of the tissue were collected from the caudal lobe of the right lung. 10µm cryostat sections were processed for the sucrosepotassium phosphate-glyoxylic acid technique to determine the occurrence and distribution of noradrenergic nerve fibres, while AChE-positive nerves were detected according to the acetylcholinesterase histochemistry. The present results revealed a dense network of noradrenergic nerve fibres localised mainly in the muscular membrane surrounding the epithelium of the bronchuli while AChE- -positive nerve terminals supplied functional capillary vessels localised in the inter-alveolar septum and mucous membrane of the bronchi and bronchuli. The results of the present study confirm those of physiological experiments reporting the influence of the adrenergic and cholinergic components of the autonomic nervous system on the lung functions of pigs.

In recent years significant progress has been witnessed in the identification of stem cells, which have now also been identified in the lungs. The aim of this was to induce post-pneumonia alveolar regeneration to facilitate the identification of stem cells. The studies were performed on Buffalo strain rats. Pneumonia was induced in the animals by a sub-pleural injection of carragenin. On days 4, 5 and 10 of the experiment both the control and experimental animals received intraperitoneal injections of bromodeoxyuridine (BrdU). Twenty-four hours after the last BrdU injection the rats were sacrificed and samples of the lungs were taken for examination. In order to detect proliferating cells in the paraffin sections, BrdU incorporation was detected with monoclonal antibodies. In pilot experiments BrdU incorporation was demonstrated in individual alveolar cells of variable distribution and of variable intensity in the colour reaction. The results have confirmed the existence of stem cells in pulmonary alveoli but their closer characterisation requires further studies with other techniques to detect pulmonary stem cells.

ObjectivesThe purpose of this study was to characterize a new respiratory syndrome associated with exposure to a humidifier disinfectant (HD) in South Korea that is distinct from the well-known HD-related lung injury (HDLI). The authors identified this condition in 24 study subjects who were family members of patients with definite or probable HDLI (referred to as index cases), and were exposed to HD in the same room as the index cases.Material and MethodsThe authors reviewed medical records of 236 family members in 110 families who were exposed to HD in the same rooms and residences as the index cases.ResultsThey identified 24 family members who were exposed to HD in the same rooms and residences as the index cases, and who developed respiratory disorders that were distinct from HDLI. The clinical signs and symptoms of these individuals were in the upper respiratory tract, such as allergic rhinitis and croup, or in the lower respiratory tract, such as bronchitis and pneumonia. The diffusing capacity of the lung fordetermicarbon monoxide was reduced in 9 of 12 children (data not available for 1 child), and in 4 of 5 adults (data not available for 6 adults). The percent forced vital capacity and forced expiratory volume in one second/forced vital capacity ratios were within the normal ranges in most patients. The computed tomography findings, which mostly indicated non-specific inflammation, were inconsistent with the radiological diagnostic criteria for HDLI, but were abnormal in 8 of 11 adults, and in 2 of 13 children.ConclusionsThe authors propose a new condition, i.e., HD-related respiratory syndrome (HDRS), which is characterized by mild to moderate or atypical respiratory symptoms and signs, and is related to HD exposure, but is distinct from HDLI. The recognition of HDRS may provide a basis for understanding the natural history of HD-related respiratory problems, and for capturing the whole spectrum of HD-related clinical manifestations in the respiratory tract.
