Dysfunction of lumbo-pelvic area is a common problem in many sports. Due to insufficient data supporting variables predisposing to lumbo-pelvic dysfunction, and a lack of standards in thorough assessment, understanding the overall problem continues to provide clinical complications; and unfortunately, the prognosis is less than promising. Most often this type of non-contact injury in the groin area is seen in dynamic sports involving running, sprinting, and sports performed over longer time periods, where fatigue plays an important role (soccer, rugby, hockey). There have been proposals in identifying most probable factors influencing the occurrence of pelvic overload injury. Among those we can list: muscle strength and balance, training regimen (including warm-up), fatigue, flexibility, body mechanics, sports specific activities, movement technique, previous injury, and psychological state. During clinical assessment by the physician and physiotherapist, many of these risks of injury factors are found. Currently more attention is given to neuromuscular factors that can affect risk of this pathology.Our clinical experience suggests that poor neuromuscular control and lack of strength may significantly contribute to injury in the lumbo-pelvic and groin area. Certain objective indexes (e.g., hamstrings to quadriceps strength H/Q, stability deficits) can be important indicators of injury risk, as well as guidelines for motor dysfunction recovery.

Every year, the number of anterior cruciate ligament ruptures increases considerably. The present analysis compares patients’ subjective sensations after the ACL injury to an objective arthrometric examination. The subjective evaluation of the knee-joint condition was performed using the Lysholm score scale and IKDC 2000 scale, whereas the objective evaluation was performed using a GNRB device to obtain an arthrometric measurement. Both measurement methods were performed before ACL reconstruction and after the rehabilitation process of ±11.5 months. The following research was conducted on a group of 8 patients who underwent anterior cruciate ligament reconstruction. Statistical analysis did not reveal any significant differences between the arthrometric examination and the subjective score scales. The test indicated the significant differences between the score scales questionnaires completed before and after the surgery. The conclusions that can be drawn from the present analysis show us the importance of using subjective evaluation devices. Despite the lack of significance in the examination performed with the objective device, the self-evaluation of knee-joint efficiency made by the patient indicated a significant improvement.

The term “proprioception” is defined as the conduction of sensory information deriving from proprioceptors that have an impact on conscious sensations, posture and trans-segmental sense. An ACL injury may lead to functional knee joint instability. According to research, this may result in impaired movement sensation and joint position. The purpose of this study was to evaluate the joint position sense (JPS) in patients before arthroscopic ACL reconstruction and 5 months after the surgery. The examinations were conducted in a group of twelve specifically selected male patients. The examination procedure consisted of JPS measurement in both lower limbs (the operated and the healthy one) during active extension in a range of angles: 30, 45, 60°. The level of significance was: p < 0.05. The analysis of variance performed for repeated measurements (ANOVA) did not indicate any statistically significant differences of JPS in comparisons made between the operated and the healthy limb. Statistical values for the absolute, relative, and variable errors were p = 0.7684, p = 0.1546, p = 0.5694 respectively. The obtained results do not indicate any limitation of proprioception in patients with ACL injury before the intervention or half a year later.

Any new method of treatment is associated with high expectations for its success, particularly if the therapy is based not only on the premise of achieving a symptomatic effect, but also improving functional quality and repairing structurally damaged tissues. Platelet Rich Plasma (PRP) application was shown to be a successful catalyst in the healing process for a wide variety of conditions in animal and human models. However, its use has been controversial due to many types of the PRP definition, optimal concentration, and modalities of implementation. In the qualification of patients for PRP treatment, not only should medical indications be considered, but also the role of participation in therapy with a physiotherapist supervising physical parameters and techniques used during recovery time. Further study is required in order to define optimal handling procedures of PRP injection. Long-term follow up will reveal if the promise of this substance can be realized and implemented to maximize its potential as a therapeutic remedy.

INTRODUCTION: The aim of this study is to demonstrate the relationship between functional shortening of the rectus femoris muscle, hamstring muscle and knee proprioception in patients after anterior cruciate ligament [ACL] rupture. MATERIAL AND METHODS: A group of 35 men with ACL rupture (without ACL reconstruction), aged 18 to 43 years (28 ± 7.4 years) was enrolled in the study. Firstly, the functional shortening of above mentioned muscles was evaluated in both healthy and injured lower limbs. Knee joint proprioception was assessed using a BTE Primus RS by examining the angular deviation from the center of the kinetic range in the knee joint with and without visual inspection. RESULTS: A statistically significant positive correlation was found between the functional shortening of the hamstrings and the proprioception of the flexion motion assessed under conditions without visual inspection. In addition, a statistically significant negative correlation was found between the functional shortening of the rectus femoris muscle of the injured lower limb and proprioception of the knee flexion movement assessed under conditions without visual inspection. CONCLUSIONS: A deficit of knee proprioception correlates with the functional shortening of the thigh muscles. Pa-tients with functional shortening of the rectus femoris have better knee proprioception. Patients with functional shortening of hamstrings have worse knee proprioception.

WSTĘP: Celem badania jest wykazanie związku między skróceniem czynnościowym mięśnia prostego uda i mięśni kulszowo-goleniowych a kinestezją stawu kolanowego u pacjentów po zerwaniu więzadła krzyżowego przedniego [ACL]. MATERIAŁ I METODY: Badaniem objęto grupę 35 mężczyzn z zerwaniem ACL (bez rekonstrukcji) w wieku od 18 do 43 lat (28 ± 7,4 roku). Funkcjonalne skrócenie wyżej wymienionych mięśni oceniano zarówno na zdrowej, jak i kontuzjowanej nodze. Kinestezję stawu kolanowego oceniano za pomocą aparatu BTE Primus RS, badając odchylenie kątowe od środka zakresu ruchu w stawie kolanowym z i bez kontroli wzroku. WYNIKI: Stwierdzono statystycznie istotną dodatnią korelację między funkcjonalnym skróceniem mięśni kulszowo- -goleniowych a kinestezją dla ruchu zgięcia ocenianego w warunkach bez kontroli wzroku. Ponadto stwierdzono istotną statystycznie ujemną korelację między funkcjonalnym skróceniem mięśnia prostego uda nogi uszkodzonej a kinestezją ruchu zgięcia kolana ocenianego w warunkach bez kontroli wzroku. WNIOSKI: Deficyt propriocepcji kolana koreluje z funkcjonalnym skróceniem mięśni uda. Pacjenci z funkcjonalnym skróceniem mięśnia prostego uda mają lepszą kinestezję stawu kolanowego, natomiast u pacjentów z funkcjonalnym skróceniem ścięgien podkolanowych stwierdzono gorszą kinestezję stawu kolanowego.

Overweight or obesity which often develop in athletes who end their careers is one of the most important issues of the world of sport. During aging, an increase in the amount of intra-abdominal fat is observed; this fat increases the risk of such metabolic disease as diabetes or hypertension. The participants of the study were 94 males. The people were randomly assigned to one of 2 groups: group 1 – people using the diet based on calorie restriction CRON (n = 32), and group 2 – those who followed the recommendations of the Mediterranean diet (n = 34). The patients who at the check-up declared not following any diet were assigned to the control group (n = 28). A change in parameters was observed in each of the sub-groups after the intervention. The most significant changes were observed in the sub-group which lost >2.6 kg. Both the diet based on mild calorie restriction and the Mediterranean diet had positive effect on the change of the former athlete’s body parameters after the 6-week long dietary intervention. Both diets which were being tested changed the body composition of the athletes who ended their careers in a similar way.

We hypothesized that the ACE ID / ACTN3 R577X genotype combination was associated with sprint and endurance performance. Therefore, the purpose of the present study was to determine the interaction between both ACE ID and ACTN3 R577X polymorphisms and sprint and endurance performance in swimmers. Genomic DNA was extracted from oral epithelial cells using GenElute Mammalian Genomic DNA Miniprep Kit (Sigma, Germany). All samples were genotyped using a real-time poly- merase chain reaction. The ACE I/D and the ACTN3 R577X genotype frequencies met Hardy-Weinberg expectations in both swimmers and controls. When the two swimmer groups, long distance swimmers (LDS) and short distance swimmers (SDS), were compared with control subjects in a single test, a significant association was found only for the ACE polymorphism, but not for ACTN3. Additionally, four ACE/ACTN3 combined genotypes (ID/RX, ID/XX, II/RX and II/XX) were statistically significant for the LDS versus Control comparison, but none for the SDS versus Control comparison. The ACE I/D and the ACTN3 R577X polymorphisms did not show any association with sprint swimming, taken individually or in combination. In spite of numerous previous reports of associations with athletic status or sprint performance in other sports, the ACTN3 R577X polymorphism, in contrast to ACE I/D, was not significantly associated with elite swimming status when considered individually. However, the combined analysis of the two loci suggests that the co-occurrence of the ACE I and ACTN3 X alleles may be beneficial to swimmers who compete in long distance races

Introduction: The anterior cruciate ligament (ACL) is one of most important structures in the knee joint. It contains mechanoreceptors, that respond to mechanical pressure or distortion and enable one to feel the knee position and proprioreception. Injury of this structure can lead to changes in knee function. Issues which relate to knee translation and knee effusion are widely described in scientific literature but increasingly more attention is being paid to muscle function: strength and stiffness. ACL trauma is often accompanied by a sensation of instability and pain during knee load. This is accompanied by joint effusion, which gradually limits the knee range of motion, exacerbates the pain and secondarily has an impact on muscle function. Functional shortening of the hamstrings can be a symptom of inappropriate control in the knee joint. Material and methods: The study was carried out in a group of 46 men aged 18–46 years old, with a mean age of 25 years old (SD = 6) with an ACL injury, which was stated after 5–52 weeks from the injury, a mean of 20 weeks (SD = 15). The functional length of the hamstring and rectus femoris muscle were measured both passively and actively by a Saunders inclinometer which allows measurement with precision up to 1°. Results: Statistical analysis revealed a statistically significant negative correlation between functional shortening of the hamstring and rectus femoris muscle in the healthy extremity, in active and passive tests. Conclusion: With the increase in functional shortening of the hamstring, functional shortening of the rectus femoris muscle is diminished, both in active and passive tests in the healthy and ACL injured extremity.

WSTĘP: Więzadło krzyżowe przednie jest jedną z ważniejszych struktur stawu kolanowego. Zawiera mechanoreceptory, które dzięki wrażliwości na ruch umożliwiają odczuwanie pozycji stawowej oraz kinestezji. Uszkodzenie powoduje zmiany w funkcji kolana. W literaturze szeroko opisywane są pozycje dotyczące translacji oraz wysięku, ale coraz większą uwagę zwraca się na funkcję mięśni: siłę i sztywność. Urazowi towarzyszą uczucie niestabilności kolana i ból przy obciążeniu kończyny urazowej. Występuje również wysięk, który stopniowo ogranicza zakres ruchu w stawie, wzmaga dolegliwości bólowe oraz wtórnie może wpłynąć na funkcje mięśni. Skrócenie funkcjonalne mięśni kulszowo-goleniowych może być objawem nieprawidłowej kontroli w obrębie stawu kolanowego. MATERIAŁ I METODY: Badania zostały przeprowadzone na grupie 46 mężczyzn w wieku 18–46 lat (średnia wieku 25 lat; SD 6), u których stwierdzono zerwanie więzadła krzyżowego przedniego (anterior cruciate ligament – ACL) po 5–52 tygodniach od urazu (średnia 20 tygodni; SD 15). Dokonano pomiaru długości czynnościowej mięśni kulszowo-goleniowych oraz prostego uda zarówno w sposób bierny, jak i czynny. W tym celu użyto inklinometru Saundersa, który pozwala dokonać pomiaru do 1°. WYNIKI: Analiza statystyczna wykazała istotną statystycznie ujemną korelację między skróceniem funkcjonalnym mięśni kulszowo-goleniowych a prostym uda w kończynie zdrowej w badaniu czynnym oraz biernym. WNIOSKI: Wraz ze wzrostem funkcjonalnego skrócenia mięśni kulszowo-goleniowych zmniejsza się skrócenie funkcjonalne mięśnia prostego uda zarówno w badaniu czynnym, jak i biernym w kończynie zdrowej. Wzrostowi skrócenia funkcjonalnego mięśni kulszowo-goleniowych towarzyszy spadek skrócenia mięśnia prostego uda w kończynie z zerwanym ACL zarówno w badaniu czynnym, jak i biernym.

Alpha-actinins are an ancient family of actin-binding proteins that play structural and regulatory roles in cytoskeletal organization. In skeletal muscle, α-actinin-3 protein is an important structural component of the Z disc, where it anchors actin thin filaments, helping to maintain the myofibrillar array. A common nonsense polymorphism in codon 577 of the ACTN3 gene (R577X) results in α-actinin-3 deficiency in XX homozygotes. Based on knowledge about the role of ACTN3 R557X polymorphism in skeletal muscle function, we postulated that the genetic polymorphism of ACTN3 could also improve sprint and power ability.We compared genotypic and allelic frequencies of the ACTN3 R557X polymorphism in two groups of men of the same Caucasian descent: 158 power-orientated athletes and 254 volunteers not involved in competitive sport.The genotype distribution in the group of power-oriented athletes showed significant differences (P=0.008) compared to controls. However, among the investigated subgroups of athletes, only the difference of ACTN3 R577X genotype between sprinters and controls reached statistical significance (P=0.041). The frequencies of the ACTN3 577X allele (30.69% vs. 40.35%; P=0.005) were significantly different in all athletes compared to controls. Our results support the hypothesis that the ACTN3 577XX allele may have some beneficial effect on sprintpower performance, because the ACTN3 XX genotype is significantly reduced in Polish power-oriented athletes compared to controls.This finding seems to be in agreement with previously reported case-control studies. However, ACTN3 polymorphism as a genetic marker for sport talent identification should be interpreted with great caution.

The purpose of this study was to examine the association of the BstUI RFLP C/T (rs 12722) and DpnII RFLP C/T (rs 13946) COL5A1 polymorphisms, individually and as haplotypes, with anterior cruciate ligament ruptures in recreational skiers. Subjects were 138 male recreational skiers with surgically diagnosed primary anterior cruciate ligament ruptures. The control group consisted of 183 apparently healthy male recreational skiers, who were without any self-reported history of ligament or tendon injury. DNA was extracted from buccal cells donated by the subjects and genotyping was carried out using real-time PCR. The genotype distributions for both polymorphisms met Hardy- Weinberg expectations in both groups. There were no significant differences in genotype distribution of allele frequencies of COL5A1 BstUI RFLP C/T and COL5A1 DpnII RFLP C/T polymorphisms between the ACL rupture and control groups. The T-T (BstUI RFLP T, DpnII RFLP T) haplotype was the most common (55.6%). The haplotype T-C was not present in any of the subjects. There was an underrepresentation tendency of the C-T haplotype in the study group compared to controls under recessive mode of inheritance. Higher frequency of the COL5A1 BstUI RFLP C/T and COL5A1DpnII RFLP C/T polymorphisms haplotype is associated with reduced risk of anterior cruciate ligament injury in a group of apparently healthy male recreational skiers.
