In this article, based on the results of questionnaire sent to 50 companies with different employment size, events affecting the failures of the ERP system implementation were identified and their risk level as well as additional costs related to preventive actions (reducing the probability or effects of the problem occurrence) were investigated. To evaluate the risk values of chosen ERP system implementation tasks, PMI (Project Management Institute) standard was applied. (original abstract)
