The article describes the course of the production process of furniture for a system with aserial structure. A two-month analysis of the production system was carried out, queues at individual machines during production, total time of execution of orders, use of machines, average production capacity have been determinded, as well as the factors that affect the delay in production were identified. A layout of the production hall was also made. All defects on machines within 2 months were also noted. The documentation and analysis of these activities allowed us to assess the risks and losses and propose a way to reduce the risk in order to improve the company's production system.

In the article we assessed the in-plant transport on the soldering line. The assessment was based on three-month process observation, registration of non-conformities and a modified PFMEA analysis form. Among the 9 non-compliance groups, the so-called critical inconsistencies for which the reasons for their generation have been determined. Actions have been developed to eliminate the causes of non-compliance, aimed at improving the process and devices of in-house transport. The proposed solutions are implemented in the analyzed organization.

This leads to the assessment of the condition of the existing supply processes in the selected coal mine and the determination of possible solutions relating to the improvement of supply logistics activities.

Prowadzi to do oceny stanu istniejącego procesów zaopatrzenia w wybranej kopalni węgla kamiennego oraz określenia możliwych rozwiązań odnoszących się do poprawy działań w zakresie logistyki zaopatrzenia.

The paper continues the study of the same authors, who previously proposed a method of risk assessment in inland navigation. In the literature there is a gap in the research area of a risk management of transportation systems, especially: inland navigation. The authors carried out interdisciplinary research and presented the results related to the identification of risk factors present in the systems of inland navigation. The paper presents inland navigation risk analysis, conducted using the linguistic variables and the FMEA method, taking into account technical, economic and social aspects. The aim of the article is to present a procedure for the assessment of risk in inland waterways transport, and carry out risk analysis for transport companies. In paper the proposal of behavior scenarios, methods of preventing and minimizing effects of pointed risks are shown.

W pracy dokonano charakterystyki transportu wewnętrznego występującego w przedsiębiorstwie produkującym cukier. Przedstawiono proces produkcyjny cukru ze szczególnym uwzględnieniem urządzeń transportu wewnętrznego. Wykorzystano dane pochodzące z tego przedsiębiorstwa uzyskane za pomocą metody obserwacyjnej i badania dokumentów oraz zebraną literaturę. Dodatkowo przeprowadzono analizę FMEA transportu wewnętrznego.

Paper characterizes the internal transport occurring in a sugar-producing company. The production process of sugar is presented with special regard to internal transport equipment. The data from this enterprise were obtained using the methods of observation and examination of documents and using the collected literature. In addition, an FMEA analysis of internal transport was carried out.

The paper presents an analysis of the defects and failures of a gear pump using the FMEA matrix analysis and error diagram. For each pump component, functions that are performed and potential defects were identified. Failures and defects that are typical for gear wheels were found as the main problem that influences pump malfunction. Therefore, using the error diagram, the main categories of causes were determined. In the next step, the identified causes and subcauses were assigned to defects with the highest possibility of occurrence, which allowed us to specify preventive and corrective actions.

W pracy przedstawiono analizę wad i uszkodzeń pompy zębatej przy wykorzystaniu macierzowej analizy FMEA i diagramu błędów. Dla każdego analizowanego elementu pompy określono funkcje, jakie pełnią w pompie i potencjalne wady, jakie mogą wystąpić. Dla analizowanej pompy wady i uszkodzenia kół zębatych określono jako główną przyczynę wadliwej pracy. Wykorzystując diagram błędów, dokonano określenia głównych kategorii przyczyn, które następnie przypisano do wad o największym prawdopodobieństwie wystąpienia. Określono również zakres działań prewencyjnych i korygujących.

The paper presents the concept of using the RCA method to failure analysis on the example of power plant. Nowadays, where the speed plays a big role in business, it’s not easy to get to the root causes of failure. RCA Method (Root Cause Analysis) is an analysis of root causes, it is an independent and comprehensive method of risk analysis used to study, identify and classify the root causes of incidents establishment in the work. The aim of the publication is to examine the benefits of using the analysis of RCA as a reactive method of identifying the causes, discovering problems and solving them in the company.

W publikacji przedstawiono koncepcję wykorzystania metody RCA do analizy awarii na przykładzie elektrowni. W dzisiejszych czasach gdzie pośpiech gra wielką rolę mało kto próbuje dotrzeć do przyczyn źródłowych awarii. Metoda RCA (Root Cause Analysis ) jest analizą przyczyn źródłowych, jest ona niezależną i kompletną metodą analizy ryzyka, służącą do badania, identyfikacji oraz sklasyfikowania przyczyń źródłowych zdarzeń zagrażających zakładowi pracy. Celem publikacji jest zbadanie korzyści płynących z zastosowania analizy RCA jako reaktywnej metody identyfikowania przyczyn, odkrywania problemów i ich rozwiązywania w firmie.

W artykule omówiona została analiza przyczyn i skutków wad w procesie logistycznym dostawy surowców, zgodnie z wytycznymi podręcznika referencyjnego AIAG. Przedstawiono mapę analizowanego procesu i zidentyfikowano czynniki wpływające na proces. Zgodnie z przyjętymi kryteriami oceny wyliczono liczbę priorytetową ryzyka dla zdiagnozowanych niezgodności i zaproponowano działania korygujące celem ograniczenia wielkości ryzyka.

The article discussed the analysis of the causes and effects of defects in the selected logistics process. It showed the process map and identified the factors influencing the process. It established evaluation criteria and calculated the risk priority number for the diagnosed failures. It proposed corrective actions to reduce the size of the risk.

Zastosowanie analizy FMEA na przykładzie małego przedsiębiorstwa kaletniczego pokazuje, że niezależnie od wielkości każe przedsiębiorstwo może skorzystać z narzędzi zarządzania jakością. Znacząco może to poprawić cały proces produkcyjny, zachęcić do współpracy pracowników wszystkich szczebli, oraz w szybki sposób informować kierownictwo o występujących problemach, co pozwoli na szybką reakcję i zdecydowanie efektywniejszą produkcję.

The use of FMEA analysis on the example of a small company demonstrates that regardless of the size of orders the company can benefit from quality management tools. Can significantly improve the entire production process, to encourage the co-operation of employees at all levels, and to quickly inform management about any problems, which will allow for quick response and definitely more efficient production.

The article presents the risk assesment of damages occurance in production stage of a babycarriage with a bike function. The analysis of risk is done by using the FMEA – Failure Made and Effect Analysis that is used in aviation and automotive industry. This way of innovative product assessment allows to evaluate the risk of defects and indicate the priorities of quality for key parts as well as the device.

W referacie omówiono zastosowanie metody macierzowej analizy FMEA. Przedstawiono przykładową analizę dla turbosprężarki doładowania silnika samochodu osobowego przy pomocy programu komputerowego opracowanego w Instytucie Informatyki Stosowanej Politechniki Krakowskiej. Uzyskane wyniki przedstawiono w postaci graficznej. Pozwoliło to w łatwy sposób dokonać interpretacji wpływu poszczególnych wad na funkcje spełniane przez wyrób.

This paper presents an application of matrix method In FEMA analysis of turbocharger for passenger car engine. Matrixes "component-failure case" and "function-component" were built. Then they were analyzed what allowed to obtain information about probability of failure for given function. Furthermore, simulation for various events was carried out to obtain smaller probability of failure by computer software made in Cracov University of Technology.
