Every day human bones undergo different injuries resulting from public transport accidents, diseases or trauma. Consequently, they cause temporary or constant patients disability or even death. Fractures happen not only to elderly people suffering from osteoporosis, but also more and more frequently to young people, who got some injuries as a result of life style, e.g. doing sports. The desire to enhance the physical well being and quality of patients' life, who got some injury causes constant development of medical devices for osteosynthesis. In the case of complicated fractures, in which plaster treatment is not sufficient, surgical treatment and applying implant are indispensable. The aim of the article is to present two case studies on intramedullary nails. The first one concerns a method of osteosynthesis for a patient, who was injured in an accident. The second one presents the failure of the implanted intramedullary nail. The research included evaluation of the type of fracture and the analysis of microstructure, chemical composition and hardness of the implant materials.

An implant is composed of different metals, such as: nickel, chromium, cobalt, titanium, molybdenum and vanadium. Some metals may cause eczema like rash or scattered vesiculopapular dermatitis and urticaria. Allergies may result in loosing the implants, bone joints and orthopedic prosthesis. The preliminary research results carried out on the patients with eczema to the components of implants. The research was performed in Hospital in Inowroclaw and it is included in master thesis of a graduate from Technical University of Gdansk.

The surgical implants are made of different materials, such as cobalt alloys, titanium alloys and stainless steels. These materials contain little amount of carbon and significant amount of Cr, Ni and Mo. The metallic implants introduced into the human body undergo degradation mainly as a result of corrosion after a long use, due to aggressive biological impact between the implant surface and human body. This phenomenon is accompanied by the release of metal ions from implant surface, which gather in tissues surrounding the implant as well as the soft tissues (for eg. the liver).

The Gamma nail (fixator) made of stainless steel, which has broken in the body, was subjected to examination. The fixator was removed from the patient femur. The fracture occurred in the area of the nail hole after two months' stay in the organism. The research included chemical composition, hardness of the Gamma nail material and the obseryation and analysis of the type of fracture. The research showed the fatigue failure of the Gamma nail.

The problems concerning re-operatiotis after joint implantation, as well as the occurrence of post-operation complications with trauma-orthopedics patients, who were given metal implants are constantly considered from medical and economical point of view. The most frequent reason for the re-operation is so called: aseptic loosening arthroplasty. The article presents the analysis of various clinical post-operation complications after the long-bone treatment by means of osteosynthesis with the use of metal implants and hip arthroplasty.
