W artykule przedstawiono charakterystykę transportu kolejowego z uwzględnienie przewozów pasażerskich oraz towarowych. Zaprezentowano podmioty odpowiedzialne za bezpieczeństwo sektora kolejowego, wśród których wyróżniono Urząd Transportu Kolejowego, spółkę PKP PLK S.A oraz Straż Ochrony Kolei. W pracy ukazano dane statystyczne uwzględniające zdarzenia na kolei oraz opisano bezpieczeństwo przewozu towarów niebezpiecznych. Dodatkowo, w ramach referatu, przedstawiono główne problemy sektora kolejowego i poprzez analizę stanu infrastruktury kolejowej, gęstości sieci oraz zdarzeń i wypadków na liniach kolejowych dokonano oceny stanu bezpieczeństwa transportu kolejowego w Polsce.

The article presents the characteristics of rail transport taking into account passenger and cargo transport. The entities responsible for the safety of the railway sector have been presented, including the Rail Transport Office, PKP PLK SA and the Railway Protection Guard. The paper presents statistical data regarding events on the railway and describes the safety of dangerous goods transport. In addition, as part of the paper, the main problems of the railway sector and the analysis of the condition of railway infrastructure, network density and events were presented and accidents on railway lines, the safety of railway transport in Poland was assessed.

W artykule dokonano analizy przyczyn i okoliczności ciężkich wypadków przy pracy, których źródłem był brak skutecznego zabezpieczenia przed dostępem do miejsc niebezpiecznych. Wskazano techniczne przyczyny wypadków w aspekcie naruszeń przepisów prawa oraz normy, które wskazują rozwiązania będące właściwymi sposobami zabezpieczeń stref niebezpiecznych.

The article presents the analysis of circumstances and causes of serious accidents at work resulting from lack of effective protection against access to hazardous areas. The article indicates technical reasons of accidents in terms of violations of laws and standards that suggest appropriate methods of protecting hazardous areas. W artykule przedstawiono koncepcję wykonania komputerowego narzędzia do interaktywnego odwzorowania zdarzeń wypadkowych występujących podczas użytkowania suwnic. Prezentowana koncepcja obejmuje sposób funkcjonowania narzędzia, jego obsługę, oprzyrządowanie zewnętrzne podłączone do narzędzia oraz wytyczne odnośnie do inscenizowanych zdarzeń wypadkowych w tym narzędziu. Przedstawiono również założenia odnośnie do środowiska wirtualnego oraz interfejsu użytkownika umożliwiającego użytkowanie wirtualnej suwnicy za pomocą dwóch rzeczywistych pulpitów sterowniczych. Narzędzie zostanie wykonane w technice rzeczywistości wirtualnej, a w połączeniu z dwoma panelami sterowniczymi umożliwi zanurzenie użytkownika w wirtualnym środowisku i uczestniczenie w inscenizowanych zdarzeniach wypadkowych z udziałem suwnicy.

In the article was presented a concept of creating a computer tool to interactively present accidents occurring during the use of overhead crane. The present concept encloses way of functioning of the tool, ifs operation, external devices switched to the tool as well as indications concerning staged accidents while operating the tool. Thesis concerning virtual environment and user's interface enabling the operation of the virtual overhead crane using two control panels was presented. The tool will be developed in virtual reality technique, and together with the two control panels, it will enable the user to plunge into the virtual environment and to participate in the staged accidents of overhead cranes.

Many incidents have occurred because organisations have failed to learn from lessons of the past. This means that there is room for improvement in the way organisations analyse incidents, generate measures to remedy identified weaknesses and prevent reoccurrence: the learning from incidents process. To improve that process, it is necessary to gain insight into the steps of this process and to identify factors that hinder learning (bottlenecks). This paper presents a model that enables organisations to analyse the steps in a learning from incidents process and to identify the bottlenecks. The study describes how this model is used in a survey and in 3 exploratory case studies in The Netherlands. The results show that there is limited use of learning potential, especially in the evaluation stage. To improve learning, an approach that considers all steps is necessary.

W artykule dokonano analizy stanu technicznego piły wzdłużnej, przy obsłudze której doszo do wypadku przy pracy. Wskazano nieprawidłowości techniczne naruszające przepisy prawa z zakresu technicznego bezpieczeństwa pracy oraz przedstawiono propozycje eliminacji zidentyfikowanych zagrożeń.

The article presents the analysis of technical condition of a circular saw operating of which caused an accident at work. technical irregularities which violated labour law and work safety were pointed out. Also, some suggestions concerning elimination of identified dangers were made.
