A high in-stent restenosis rate and thrombosis have compromised clinical benefits after vascular stent placement. Exercise rehabilitation after stenting emerges as a promising and practical therapeutic strategy to improve the clinical performance of this therapy, although it remains controversial. The present study aimed to explore the impact of exercise training on hemodynamic performance after vascular stent implantation. Different 3-dimensional computational models based on the patient-specific carotids were constructed to calculate hemodynamic parameters, including flow velocity, time-averaged wall shear (TAWSS), oscillatory shear index (OSI) and relative residence time (RRT). The results demonstrated that exercise training increased TAWSS but decreased OSI and RRT in some cases after the intervention, and high-intensity exercise further suppressed the adverse blood flow. However, exercise training remarkably reduced TAWSS and elevated OSI and RRT in patients with mild stenosis at upstream of stented segment. Additionally, we discovered that the hemodynamic environment change induced by exercise training was not significant compared to the stent position in some cases. Exercise had a less beneficial impact on the disturbed blood flow after the distal common carotid artery (CCA) stenting. These findings highlighted that exercise-induced hemodynamic changes differ under different conditions. The exercise training for the intervention patients should only be performed after a comprehensive vascular function assessment.

Przedstawiono wybrane przykłady stentowania w obrębie przewodu pokarmowego ze szczególnym uwzględnieniem stentowania przełyku z wykorzystaniem wideoendoskopii. Omówiono rodzaje i właściwości stentów. Podano przykłady leczniczych aplikacji stentów w przewodzie pokarmowym. Podkreślono znaczenie jednoczesnego zastosowania różnych metod obrazowania.

Some selected examples of stenting concerning the upper digestive tract with special attention to the esophagus stenting using videoendoscopy are presented. Types and properties of stents have been described. Examples of therapeutic applications of the stents are given. The importance of the use of combined imaging is emphasized.

The subject matter of this paper concerns advanced techniques of imaging used in diagnosis and minimally invasive procedures applied in non-operable cases of the digestive tract tumour therapy. The role of optical techniques in current medical imaging is significant. Optical properties of transilluminated or illuminated tissues and organs depend on strong light absorption and scattering. Numerous issues related to the result interpretation still remain unsolved. Effectiveness and precision can be especially improved when some combined methods of imaging are used. Videoendoscopy imaging, X-ray imaging, and endoscopic ultrasound imaging are three complementary methods applied during the interventions described in this work. All interventions have concerned with the upper part of the digestive tract. Especially, interdisciplinary issues of combined medical imaging are presented using some examples of a modern approach to imaging of esophagus and biliary stenting. The selected examples of effects obtained during interventions assisted by combined imaging of the operation site are presented. The operator can have great control over the appearance of desirable effects as well as undesirable complications in order to work comfortably and safe.
