Early Soviet medicine: statistical and narrative utopias

Having found confl icting versions of the past in publications on the history of Soviet medicine, the authors of the article problematized the evidence with which historians work. This led to the study of the production and interaction of statistical and narrative statements of the health care authorities of the 1930s, that is, their reporting and futuristic pipe dreams. The comparison of the medical statistics published in the official directories and the current reporting of medical institutions revealed discrepancies between the published and collected information. Criticism of the official figures by contemporaries gave researchers the opportunity to reveal material and construction technologies of a utopian reality, from the power of which even modern researchers find it hard to free themselves.

Napotkawszy sprzeczne wersje przeszłości w publikacjach poświęconych dziejom radzieckiej medycyny, autorzy artykułu podeszli do problemu świadectw, z którymi pracują historycy. Doprowadziło to do przebadania tworzenia i wzajemnego oddziaływania twierdzeń statystycznych i opisowych, tworzonych przez władze służby zdrowia w latach trzydziestych XX w. - czyli ich sprawozdań i snów o potędze. Porównanie statystyk medycznych publikowanych w oficjalnych dokumentach i bieżących raportów instytucji medycznych ujawniło rozbieżności między zebranymi a ogłoszonymi informacjami. Ówczesna krytyka oficjalnych danych dała badaczom możliwość ujawnienia materiałów i technik konstrukcji utopijnej rzeczywistości, od której jest trudno uwolnić się nawet współczesnym badaczom.

Grade Approach to the Analysis of Questionnaires and Clinical Scales Data

Grade approach has been applied to visualize and reinvestigate a dataset from diagnostic questionnaires and scales used in psychological and psychiatric clinical practice. Grade exploration reveals important hidden variables strongly influencing other variables and splits questionnaire data into more regular monotone dependent blocks. Two sets of homogeneous and ordered clusters of patients, formed on the basis of first line variables and described also by second line variables, expose links between neurosis symptoms and Internal or External Locus of Control as well as prove useful for specifying and verifying diagnoses. All analyses were performed using the GradeStat program.

Non-linearity and statistics - implications of hormesis on dose-response analysis

In medical applications of statistics, such as drug testing or risk assessment, most investigators usually expects linear dose-response pattern and are rather surprised finding that the effects on some individuals are even opposite than those expected from the assumption of linearity. We provide several examples of non-linear dose-response pattern, the phenomenon called hormesis, both chemical and radiation hormesis. We also discuss possible explanation of hormesis based on cybernetic regulation mechanism or on chaotic attractor hypothesis, as well as consequences of hormesis for epidemiology and medical statistics.

W statystyce medycznej, na przykład przy testowaniu leków lub ocenie ryzyka, większość badaczy oczekuje liniowej zależności wyniku działania od zastosowanej dawki i często zaskoczona jest faktem, że wynik bywa wręcz przeciwny niż oczekiwany przy takim założeniu. W pracy zaprezentowano kilka przykładów niełiniowej zależności wyniku działania od dawki, zjawiska zwanego hormezą, zarówno hormezy chemicznej jak też hormezy radiacyjnej. Przedyskutowano także próby wyjaśnienia zjawiska hormezy oparte na cybernetycznym modelu regułacji i hipotezie chaotycznego atraktora oraz konsekwencje zjawiska hormezy dla epidemiologii i statystyki medycznej.
