The main objective of this study was to analyze anomalies voluntarily reported by pilots in civil aviation sector and identify factors leading to such anomalies. Experimental data were obtained from the NASA aviation safety reporting system (ASRS) database. These data contained a range of text records spanning 30 years of civilian aviation, both commercial (airline operations) and general aviation (private aircraft). Narrative data as well as categorical data were used. The associations between incident contributing factors and selfreported anomalies were investigated using data mining and correspondence analysis. The results revealed that a broadly defined human factors category and weather conditions were the main contributors to selfreported civil aviation anomalies. New associations between identified factors and reported anomaly conditions were also reported.

The paper describes using the James Reason model in investigation aircraft accidents. According to statistic human error is the main factor both military and civil aviation accidents. The causes of the accidents are usually referred as "pilot error". The James Reason model used during the investigation process allows to find other failures than crew unsafe acts like: preconditions for unsafe acts, unsafe supervision and/or organizational (management) influence. That findings allow the investigators take corrective action not only to mishap crew but to the whole system including procedures, training, regulations and even aviation law etc"

Artykuł przedstawia wykorzystanie modelu "Jamesa Reason" w badaniu wypadków lotniczych. Według aktulnych statystyk błąd człowieka jest główna przyczyną wypadków lotniczych. Najczęściej przyczynę wypadku określa się jako "błąd pilota" lub "błąd załogi". Wkorzystanie modelu "Jamesa Reason" pozwala na ustalenie wielu czynników mających wpływ na niewłaściwe działanie załogi takich jak: wyrunki sprzyjające do niebezpiecznego dzaiłąnia załogi, niewłaściwy nadzór czy zarządzania organiazacją lotniczą. Pozwala to prowadzić działania profilaktyczne nie tylko w stosunku do załogi statku powietrznego ale również w stosunku do całego systemu w tym: procedur, szkolenia, przepisów i nawet prawa lotniczego.
