One of the most important issues, when discussing creation of rational health care system, is improving its efficiency. One of the solutions is for example computerisation of the health care, which includes creation of a national medical services register - RUM - that is a system of collecting and processing medical services data. The basic assumptions of launching RUM occurred in the beginning of the 90's. Some of Health Insurance Fund have already implemented RUM. Nowadays the experience gained in the past is being used in works leading to launching the system RUM II. NFZ is responsible for the coordination of program of implementation and has set a "Strategy of using databases by NFZ and the directions of NFZ data system development for the years 2001-2010" as well as "Conception of RUM II generation". Their assumptions and predicted results and benefits will be discussed in this analysis.

Celem pracy jest przedstawienie aspektów technologicznych, prawnych i uwarunkowań organizacyjnych oraz zdrowotnych, jakie są oczekiwane i dyskutowane w środowisku naukowym, medycznym, informatycznym odnośnie do informatyzacji w sektorze ochrony zdrowia. Rozważania dotyczą efektów wieloletniej informatyzacji placówek medycznych, systemu rejestru zdarzeń medycznych, potocznie nazywanych RUM, oraz nowego trendu w dyskusji nad budową platformy gromadzenia, analizy i udostępniania zasobów cyfrowych o zdarzeniach medycznych, która obejmuje zagadnienia związane z tworzeniem tzw. Elektronicznego Rekordu Pacjenta - ERP (ang. Electronic Health Record - EHR). Artykuł odnosi się do poglądów i możliwości IKT, których podstawą jest realizacja Parlamentu Europejskiego w sprawie mobilności pacjentów i zjawisk zachodzących w ochronie zdrowia w Unii Europejskiej. Technologie informacyjne i komunikacyjne, zwłaszcza internet, mogą pomóc w ułatwieniu dostępu oraz w poprawie jakości i skuteczności służby zdrowia, chociaż mogą także zwiększyć nierówności w konsekwencji wykluczenia na skutek luki technologicznej (ang. digital divide).

The aim of work is to present technological and legal aspects, as well as organizational conditions that are expected and discussed among scientists, medical and computer science specialists, regarding the health care sector. These considerations concern the effects of long-term informatization of medical institutions, medical events register system popularly known as RUM (in Polish: register of medical services), as well as the new trends concerning the platform of information gathering, analysis, as well as making the digital resources widely available, what cover issues of so called Electronic Health Record (EHR). The paper discuesses the existing views concerning information and communication technology, especially internet, and possibilities of introducing these technologies into medical system to facilitate the access to the health services, as well as to improve its effectiveness, taking into account the digital divide at the same time.

Działania związane z informatyzacją sektora zdrowia nie przyniosły dotycliczas oczelciwanycli efelctów. Przyczyn upatruje się w sytuacji politycznej państwa oraz braku odpowiednicli uregulowań w przepisach praw-nycli. Przedstawiono dwie koncepcje informatyzacji służby zdrowia, oparte na tzw. rekordzie pacjenta oraz systemie rozliczeniowym RUM (rejestr usług medycznych) oraz omówiono perspektywy rozwoju.

The previous actions concerning informatization of health sector have not brought expected effects. The political situation of Poland as well as lack of the appriopriate regulations caused this. Two conceptions of informatization of health sector, based on the patient record and clearing system (Medical Services Register) were presented as well as the prospects of development were discussed.
