[Introduction] Schedule management of hospitalization is important to maintain or improve the quality of medical care and application of a clinical pathway is one of the important solutions for the management. Although several kinds of deductive methods for construction for a clinical pathway have been proposed, the customization is one of the important problems. This research proposed an inductive approach to support the customization of existing clinical pathways by using data on nursing actions stored in a hospital information system. [Method] The number of each nursing action applied to a given disease during the hospitalization was counted for each day as a temporal sequence. Temporal sequences were compared by using clustering and multidimensional scaling method in order to visualize the similarities between temporal patterns of clinical actions. [Results] Clustering and multidimensional scaling analysis classified these orders to one group necessary for the treatment for this DPC and the other specific to the status of a patient. The method was evaluated on data sets of ten frequent diseases extracted from hospital information system in Shimane University Hospital. Cataracta and Glaucoma were selected. Removing routine and poorly documented nursing actions, 46 items were selected for analysis. [Discussion] Counting data on executed nursing orders were analyzed as temporal sequences by using similarity-based analysis methods. The analysis classified the nursing actions into the two major groups: one consisted of orders necessary for the treatment and the other consisted of orders dependent on the status of admitted patients, including complicated diseases, such as DM or heart diseases. The method enabled us to inductive construction of standardized schedule management and detection of the conditions of patients difficult to apply the existing or induced clinical pathway.

Artykuł prezentuje aplikację zarządzającą pracownią radiologiczną, która organizuje prace wykonywane przez systemy peryferyjne pracowni. Wymogiem jej działania jest poprawna komunikacja z pozostałymi systemami wykorzystywanymi w pracowni, oraz szpitalnym systemem informacyjnym. Aplikacja ma na celu usprawnienie działania placówki obsługującej dużą liczbę pacjentów, która wymaga szybkiej obsługi danych z pracowni.

The article presents a radiology lab management application, which organizes the operations performed by the peripheral systems of the laboratory. The prerequisite for its operation is its correct communication with the other systems used in the laboratory as well as the IT system of the hospital. The purpose of this application is to improve the operation of an institution handling a large number of patients and requiring quick data handling within the laboratory.

Artykuł prezentuje opis modelu przetwarzania badań, jakie wykonywane są w pracowniach radiologicznych. Wyniki wykonanych badań w postaci opisów tekstowych - standard HL7 oraz obrazów - standard DICOM powiązane są następnie ze strukturą szpitalnego systemu informacyjnego. Proponowana aplikacja zarządzająca pracowniami radiologicznymi może zostać zaimplementowana w placówce o dowolnej wielkości z wdrożonym systemem informacyjnym oraz z dowolną liczbą aktywnych pracowni obrazowania medycznego. Opracowana aplikacja może być testowana poza obrębem sieci szpitalnej.

Applications managing a radiology laboratory organize works per-formed by the laboratory peripheral systems. Proper communication with other systems used in the laboratory and a hospital IT system is required for realizing this task. The paper presents description of a model of processing the tests performed in radiology laboratories. The test results in the form of text descriptions - HL7 standard and images - DICOM standard are subsequently affiliated with the hospital system structure. The proposed application which manages radiology laboratories may be implemented in the facility of any size with the IT system implemented and with any number of active medical image laboratories. A simulator is also proposed to enable testing applications outside the hospital network. A communication model assumes the usage of HL7 and DICOM standards. Due to different versions of these protocols and large freedom during creating messages, the application uses the protocol which sends the messages in the XML format. The messages are patterned on the HL7 and DICOM equivalents, sending the same information, however encoded in the manner which is unambiguously read out by all elements of the application (Fig. 2). The creation of software which may be implemented in the productive environment requires application of the developed testing process. In the case of medical facilities the process is even longer. The next step is to prepare communication interfaces for HL7 and DICOM protocols. It will allow conducting tests with participation of the software which stimulates events signaled by means of these protocols and will be the next stage for testing the application in a real radiology laboratory.

Wymogiem jej działania jest poprawna komunikacja z pozostałymi systemami wykorzystywanymi w pracowni, oraz szpitalnym systemem informacyjnym. Aplikacja ma na celu usprawnienie działania placówki obsługującej dużą liczbę pacjentów, która wymaga szybkiej obsługi danych z pracowni.

The paper presents a radiology lab management application, which organizes activities performed by peripheral systems of a laboratory. The prerequisite for its operation is its correct communication with other systems used in the laboratory as well as the hospital IT system. The purpose of this application is to improve the institution activity handling a large number of patients and requiring quick data handling within the laboratory. The work on development of the structure of the reports involved a large emphasis being placed on the adherence to the IHE recommendations. The model put forward assumes a multiple sending process for the very same information in order to maintain synchronization. Those decisions were important from the point of view of programming the report handling procedures. The exact project of data exchange improves the process of application programming. The use of XML format for coding the information allows the data exchange to be traced with use of a sniffer. The communication mechanics adopted by us allows for efficient exchange of information. There are more than a dozen commercially available HL7 protocol interfaces. The lack of availability of a specification and the ambiguous mechanics of data organization, used within Polish hospital applications has led us to use our own protocol. The working solution utilizes only a few communicates based on HL7. The work puts a higher emphasis on the informational content of those communicates. Our own protocol is also used to code DICOM communicate equivalents, which convey text messages or serve as event reminders.

Artykuł sygnalizuje problemy oraz proponuje konkretne rozwiązania dotyczące komunikacji pomiędzy modułami systemu informatycznego wewnątrz jednostek opieki zdrowotnej. W przypadku implementacji poszczególnych modułów (lub nawet podsystemów) w szpitalach, na ogół moduły te nie mają możliwości wymiany informacji między sobą. Jest to powód małej efektywności systemów, a przede wszystkim braku komunikacji między jednostkami organizacyjnymi szpitala. Istnieje potrzeba opracowania modelu oraz standardów przesyłu informacji wewnątrz jednostek opieki zdrowotnej.

On the base of the analysis of communication problems between different parts of the hospital information systems we propose general model of the communication inside health care institution. Frequently, after implementation of different parts of the hospital information system there are several problems with communication between them. This courses low efficiency of the information system, particularly lack of communication between different organizational units. General model and standards for such communication should be proposed.
