Content available remote Development of Ergonomically Designed Functional Bra for Women with Hemiplegia

Hemiplegia is the lifelong paralysis, caused by brain stroke that affects one side of the body. Stroke exerts a great threat to patient’s health and lives, and hemiplegia after stroke may severely affect their activities of daily life including self-dressing that causes fatigue and frustration and ultimately ended with “dependence on others.” This situation damages the sense of dignity, especially for women, while independently dressing intimate apparel like bra. The research through the design approach is used as the conceptual framework to design and evaluate bra for the women with hemiplegia. Three design criteria were identified from which specifications were developed, and their interrelationships explored through an interaction matrix. This article presents a research work on improving bra design, adjusted to the special needs and demands of an individual with hemiplegia, by using bi-layer knitted fabric, magnetic fasteners, and shape-memory alloys. The findings of the current study showed that adaptive bra design offers independence, concealment of the disability, and comfort and psychological contentment. This adaptive bra promotes the harmony between functionality and esthetics.

The aim of this study was to evaluate how wearing one or two orthoses influence the gait parameters in children with hemiplegia. Four children with hemiplegic cerebral palsy participated in the study. Patients were from 3 to 7 years old; one girl and three boys; one patient with right side hemiplegia, three patients with left side hemiplegia. Patients underwent the VICON MX system gait analysis three times (sessions): one with bare feet, the other two with shoes and orthoses: one with orthoses used unilaterally on palsied limb, one with orthoses used bilaterally. The following gait parameters were analyzed: velocity, cadence, pelvis tilt, range of pelvic movement in sagittal plane, separately for affected and non-affected side: step length, stance time (as a percentage of the gait cycle), range of hip movement in sagittal plane, knee flexion at initial contact, maximum knee flexion in swing phase. Results of the study showed which and how many gait parameters changed while wearing one in comparison with two orthoses. The results did not give a clear indication which condition is better: wearing one or two orthoses, however they suggest that when the orthotic devices are considered for patients with hemiplegic cerebral palsy, the objective, instrumented gait analysis can be used to take the best decision whether one or two orthoses should be prescribed.

Content available remote Gait patterns classification based on cluster and bicluster analysis

Gait patterns of hemiplegia patients have many potential applications such as assistance in diagnosis or clinical decision-making. Many techniques were developed to classify gait patterns in past years; however, these methods have some limitations. The main goal of the study was to present the performance evaluation results of the new biclustering algorithm called KMB. The second objective was to compare clustering and biclustering methods. The study was performed based on the gait patterns of 41 hemiplegia patients over 12 months post-stroke, at the age of 48.6 ± 19.6 years. Spatial–temporal gait parameters and joint moments were measured using motion capture system and force plates. Clustering and biclustering algorithms were applied for data consisting of joint moments of lower limbs. The obtained results of this study based on joint moments, clustering, and biclustering can be applied to evaluate patient condition and treatment effectiveness. We suggest that the biclustering algorithm compared to clustering algorithms better characterizes the specific traits and abnormalities of the joint moments, especially in case of hemiplegia patients.

Purpose: Hemiplegia is a paralysis on one side of the body resulting from disease or injury to the motor centers of the brain that may lead to difficulty in walking and problems in balance. A new methodology for hemiplegia gait patterns classification based on bicluster analysis, which aims to identify a group of patients with similar gait patterns, and verify if spatial-temporal gait parameters are correlated with the Barthel Index, has been proposed. Methods: Eighteen hemiplegia patients were recruited. Measurements included spatial-temporal gait parameters and joint moments. Gait data were measured using a motion tracking system and two force platforms. Bicluster analysis was used to classify the subjects' gait patterns. The relation between Barthel Index and spatial-temporal gait parameters was determined based on the Spearman correlation. Results: A high correlation between spatial-temporal gait parameters and Barthel Index (r > .5, p < .05) was observed. Well-separated biclusters presenting similarity among the lower limb joints during the gait cycles were obtained from the data. Conclusions: Bicluster analysis can be useful for identifying patients with similar gait patterns. The relation between the gait patterns and the underlying impairments would allow clinicians to target rehabilitation strategies at the patient’s individual needs.

Celem pracy była analiza chodu osób z porażeniem połowiczym po udarze mózgu. Dokonano porównania wybranych parametrów lokomocji osób z porażeniem do chodu osób zdrowych, a także kończyny po stronie porażonej i nieporażonej. Analizie poddano 10 osób, w tym 4 kobiety i 6 mężczyzn. Wyniki badań przedstawiono na tle rezultatów chodu z naturalną prędkością młodych, zdrowych osób, stanowiących grupę porównawczą.

The aim of this paper was to analyse the gait of people after stroke with hemiplegia and make comparision with the health people’s walk. Gait parameters obtained for paralyzed limb were compared with those obtainted for health one. Ten people after strokes with hemiplegia, including 4 women and 6 men, were taken into consederation. The results were presented on the background of the walk of the young, health people who were the reference group.

U pacjentów z niedowładem połowiczym występuje znacznie zaburzony wzorzec chodu. Przeprowadzono analizę rozkładu sił i symetrii ciała przy zastosowaniu standardowego zaopatrzenia ortopedycznego podczas reedukacji chodu u pacjentów z hemiplegią. Artykuł ma na celu zwrócenie uwagi na potrzebę bardzo ostrożnego doboru zaopatrzenia ortopedycznego dla pacjentów z niedowładem połowiczym oraz przedstawia alternatywne metody postępowania.

The aim of article is to pay attention for the need of strictly carefully selection of orthopedic devices for patients with hemiparesis and to show alternative methods of coping.
