W pracy opisano zastosowanie aplikacji internetowej do oceny jakości dopasowania aparatów słuchowych. Metoda oceny polega na badaniu ankietowym, uzupełnionym testem rozumienia słów jednosylabowych w polu swobodnym. Opisywana aplikacja internetowa pozwala na przeprowadzenie badania z dowolnego komputera z dostępem do sieci. Dzięki implementacji metody w postaci aplikacji internetowej, można w systematyczny i uporządkowany sposób dokonywać oceny korzyści z użytkowania aparatów słuchowych. Daje też możliwość jej rozszerzenia czy modyfikacji. Z przeprowadzonych badań wynika, że ok. 60% użytkowników uzyskało w ciągu 7 dni zysk z aparatu słuchowego ≥10%, co oznacza efektywne zaopatrzenie. Największy zysk osiągają pacjenci z 2. i 3. stopniem niedosłuchu według skali WHO (World Health Organization).

The study presents application of the web-based application for the assessment of quality of hearing aid fitting. The method consists of a survey supplemented with the test for recognizing the single syllable words under free-field conditions. A web-based application has been developed to allow testing from any computer with the Internet access. The implementation of the method in the form of a web-based application, allows a systematic and organized assessment of the benefits gained from the hearing aid use. The application is an easy-to-use tool that can easily be modified. On the basis of the study performed, it may be observed that in the case of 60% of subjects it was possible to achieve benefit ≥10%, which signifies an effective fitting of the hearing aid. The highest benefit occurs for subjects with grade 2 and 3 of hearing loss according to the WHO scale.

This text analyses several topics connected with hearing test methods and hearing prosthetics for patients with hearing impairments or hearing loss. The hearing is one of the senses which plays a crucial part in human's cognition of the surrounding world and spatial orientation. Proper functioning of the hearing organ is a foundation of communication between people as well as the development of civil society. A hearing impairment or hearing loss disrupts social relationships between people, it entails developmental disorders in children, and causes learning difficulties. Methods of diagnosing patients and hearing prosthetics are very complex topics and are investigated by interdisciplinary research teams. The development of these fields of medicine and technology contributes to improving the quality of life for people with hearing impairment or deafness. This paper presents ways and means of improving or compensating for hearing losses with the use of hearing aids. Nowadays, hearing impairments and hearing losses concern even young people. One of the reasons for the deterioration of hearing is the ubiquitous noise in the modern world. Moreover, this text discusses the construction of hearing aids, with a particular emphasis placed on modern technological solutions. It is highlighted that hearing aids are currently available for the general public.

W pracy dokonano analizy zagadnień związanych z metodami badania i protetyki słuchu pacjentów z niedosłuchem i ubytkiem słuchu. Słuch jest jednym z receptorów, który odgrywa u człowieka ważną rolę w poznawaniu otaczającego świata i orientacji w przestrzeni. Prawidłowe funkcjonowanie narządu słuchu stanowi podstawę komunikacji między ludźmi i rozwoju społeczeństwa. Uszkodzenie lub utrata słuchu zaburza relacje społeczne między ludźmi, powoduje zaburzenia rozwoju u dzieci oraz trudności w nauce. Metody diagnozowania pacjentów i protetyka słuchu stanowią bardzo złożone zagadnienia, którymi zajmują się interdyscyplinarne zespoły badawcze. Rozwój tych dziedzin medycyny i techniki przyczynia się do poprawy jakości życia osób z niedosłuchem lub głuchotą. Przedstawiono w pracy sposoby i możliwości poprawy lub kompensacji ubytków słuchu za pomocą aparatów słuchowych. Wady i ubytki słuchu dotyczą obecnie nawet młodych ludzi. Jedną z przyczyn pogorszenia się słuchu u człowieka jest otaczający nas we współczesnym świecie hałas. Zaprezentowano też w pracy budowę aparatu słuchowego, ze szczególnym uwzględnieniem nowoczesnych rozwiązań technologicznych. Podkreślono, że aparaty słuchowe są obecnie dostępne dla ogółu społeczeństwa.

The investigations of speech intelligibility were carried out for 9 patients with a cochlear implant (CI), 1 male and 8 females, aged 18-69 (mean 41). In all cases deafness was recognized as postlingual. All patients were using the hearing aid (HA) on the ear opposite to the CI. The novelty of this work consisted in the application of new Polish sentence tests in the evaluation of the effect of supporting electrical hearing with a HA in noise conditions. The sentence tests (developed at the Institute of Acoustics, Adam Mickiewicz University in Poznań - European Union Project HEARCOM) were presented to patients in quiet and in noise conditions (speech bubble noise). The speech intelligibility was determined in two cases: when transmission was only via the CI and when the HA was also used. The presentation of tests was conducted for three angles (0°, -90° and +90°o) in relation to the patient's head. The value of SRT (Speech Reception Threshold) was determined for the tests carried out in noise conditions. The improvement of the speech intelligibility was affirmed when the process of hearing via the CI was supported with a HA. The particularly essential difference was observed for the angle of +90° (HA-side). The results obtained in the investigations show that acoustic compensation in the range of the remaining audibility field to a great degree supports electrical hearing. This conclusion can be an important contribution to the application of the HA on the side opposite to the CI, especially in the noisy environment.

The acceptable noise level (ANL) procedure was developed to quantify the maximum amount of background noise that listeners are willing to accept while listening to speech. The ANL is calculated as a difference between a listener’s most comfortable listening level (MCL) for speech and the maximum acceptable noise level (BNL). Successful hearing aid use is related to an individual’s ability to accept background noise. A formula was developed to calculate the probability of success with hearing aids as a function of individual ANL. Research has demonstrated that ANLs with and without hearing aids are highly correlated, and the ANL test can predict hearing-aid use with 85% accuracy. Several interesting issues have emerged from recent ANL investigations: 1) How should a patient be counseled regarding the ANL? 2) What factors contribute to the large inter-subject variability of ANL? 3) If successful hearing aid use relates to the ability to accept background noise, can this ability be enhanced by auditory training or pharmacological means? 4) Can hearing aids with advanced processing features, such as noise reduction, allow occasional or unsuccessful hearing aid users to become successful users?

Content available remote Contactless hearing aid designed for infants

It is a well known fact that language development through home intervention for a hearing-impaired infant should start in the early months of a newborn baby's life. The aim of this paper is to present a concept of a contactless digital hearing aid designed especially for infants. In contrast to all typical wearable hearing aid solutions (ITC, ITE, BTE), the proposed device is mounted in the infant's bed with any parts of its set-up contacting the infant's body. A processed speech signal is emitted by low-power loudspeakers placed near the infant's head. The hearing aid architecture employs a digital signal processor based on Texas Instruments technology. Since one of the main problems is the acoustic feedback between the microphone and the loudspeakers, the methods of its elimination are also briefly reviewed in this article. The first of the discussed methods employs an adaptive algorithm, the second alters the frequency response of the entire instrumentation through the use of notch filter banks, and the third incorporates a microphone array and beam-forming techniques. The paper also includes descriptions of some algorithmic solutions engineered by the authors in purpose to eliminate acoustic feedbacks. All the conclusions introduced in this article have been derived based on the simulations of an experimental contactless hearing aid set-up.

This paper presents chosen aspects of developing new DSP algorithms, which can be used in devices for hearing impaired people. We have concentrated on the advanced test for the hearing loss diagnosis and on the real time directional filtering both realized using a DSP (digital signal processor). The analyzed algorithms are implemented using specially designed, experimental DSP modules.

People with hearing impairments experience a reduction in speech intelligibility in noisy surroundings. The extent of this reduction depends on the degree of impairment in the temporal resolution of the patient's ear, too. The change of the speech intelligibility of hearing-impaired people was investigated in the presence of different masking noises, especially when using hearing aids with adjustable attack (AT) and release (RT) times within same frequency bands.
