Laser skin resurfacing: comparison of laser systems

Abstrakty
Możliwości zastosowania różnorodnych urządzeń laserowych w ostatnich 10 latach bardzo wzrosły, ze względu na postępy w rozwoju techniki laserowej. Także i w medycynie, zastosowanie laserów w różnych dyscyplinach klinicznych nie może nie być uwzględniane. Wraz z rozwojem różnych systemów, technika laserowa od kilku lat znajduje zastosowanie w polepszaniu struktury skóry w chirurgii kosmetycznej czy medycynie estetycznej. Stosując laser w tzw. laser resurfacing, można usuwać wierzchnie warstwy naskórka, nie powodując przy tym uszkodzeń termicznych. W szczególności w przypadku zmarszczek wokół oczu, ust, w fałdzie nosowym, osiąga się bardzo dobre rezultaty. Także w przypadku blizn potrądzikowych, można uzyskać widoczne polepszenie struktury skóry. Operacje są minimalnie inwazyjne, bezkrwawe i mogą być przeprowadzane w znieczuleniu miejscowym. Głębokość wnikania może być dokładnie kontrolowana. W porównaniu z dermabrazją mechaniczną czy chemiczną proces gojenia jest tu krótszy i bez komplikacji. W obecnej chwili w laser resurfacing używa się zasadniczo 3 systemów: laser CO2 z układem skanującym, laser CO2 impulsowy i laser Er:YAG. Systemy te zostaną tu zaprezentowane, a osiągnięte rezultaty kliniczne porównane.
Thanks to technical progress and improved application techniques the steadily increasing number of laser systems has rapidly become more and more versatile in application over the past ten years. This is also true for the medical sector where lasers, meanwhile, have become common standard in most clinical disciplines. Due to the development of the specific laser systems this technique has also been employed for some years already in cosmetic surgery/aesthetic medicine for improving the skin structure. Using the laser for skin resurfacing permits the upper dermal layers of the epidermis to be carefully removed up to the papillary dermis without any significant thermal damage to the adjacent tissue. This technique can be successfully applied particulary in the regions of the perioral and frontal wrinkles and the fine periorbital rhytides. Moreover, a significant improvement of the skin structure can be achieved by using laser peeling for the treatment of acne scars. The treatments are minimally invasive, without bleeding and can mostly be performed painlessly under local anasthesia. The permanent depth can be exactly controlled. Compared the chemical peeling or dermabrasion by a fraise the healing period is shorter and there is less danger of side effects. Currently, three laser systems are available on the market which are used for skin resurfacing, namely the CO2 laser or the Er:YAG laser. These there systems were compared in therms of their clinical effects.
