PL

Wiskosuplementacja jest jedną z metod nieoperacyjnego leczenia choroby zwyrodnieniowej stawu kolanowego. Polega na uzupełnianiu ilości endogennego kwasu hialuronowego w stawie za pomocą jednej lub kilku iniekcji. Zmniejsza dolegliwości bólowe pacjenta i poprawia zakres ruchomości stawu. Autorzy przeanalizowali literaturę dotyczącą badań nad wynikami stosowania wiskosuplementacji u pacjentów z gonartrozą i podjęli próbę uporządkowania wiedzy dotyczących jego stosowania. Ocenie poddano prace porównawcze działania kwasu hialuronowego vs placebo, analizy dotyczące wpływu budowy cząsteczki preparatu na efekt leczniczy, a także porównania z iniekcjami kortykosteroidowymi. Iniekcje dostawowe kwasu hialuronowego są efektywną i bezpieczną metodą w profilaktyce i leczeniu choroby zwyrodnieniowej stawu kolanowego. Zmniejszają dolegliwości bólowe oraz poprawiają funkcję ruchową u pacjentów. Notuje się niewielki odsetek działań niepożądanych po podaniu. Działanie kwasu hialuronowego nie ogranicza jedynie subiektywnych odczuć chorego. Istnieją opracowania, które wskazują na zmniejszenie degradacji chrząstki stawowej w badaniu MRI, u pacjentów stosujących wiskosuplementację. Wykazano przewagę kwasu hialuronowego nad iniekcjami kortykosteroidowymi, które chociaż dają szybszy efekt przeciwbólowy, działają krócej i stanowią czynnik uszkadzający chrząstkę stawową. Pozwalają odroczyć alloplastykę stawu kolanowego u pacjentów z zaawansowaną gonartrozą (Kellgren-Lawrence IV), a w grupie chorych poddanych artroskopii stawu kolanowego, skracają okres rekonwalescencji. Wiskouplementacja jest efektywną i bezpieczną metodą leczenia objawowej gonartrozy i stanowi uzupełnienie lub alternatywę dla stosowania innych technik terapii zachowawczej.

Viscosupplementation is one of the nonsurgical methods of treating the knee osteoarthritis. In this procedure the amount of endogenous hyaluronic acid is supplemented by means of one or several joint injections. Such a treatment relieves pain and improves the range of the joint motion. The authors analyzed the research literature concerning viscosupplemetation in patients with gonarthrosis and attempted to organize the knowledge concerning the hyaluronic acid application. The following papers were taken into consideration: comparative studies of hyaluronic acid versus placebo efficacy, analyses of the structure of the HA and its overall effectiveness, and comparison studies of hyaluronic acid vs. corticosteroid injections. Hyaluronic acid joint injections are an effective and safe method in protection and treatment of knee osteoarthritis. They relieve pain and improve the range of joint motion. There have been only few side effects noted. The application of hyaluronic acid provides subjective positive feelings in patients, yet there are also studies proving that viscosupplementation leads to the decrease of cartilage degradation, as shown in MRI tests. There is a clear advantage of hyaluronic acid over corticosteroid injections – even though the latter relieve pain faster, they act for a shorter period of time and they damage the cartilage in due course. Hyaluronic acid injections postpone the necessity of knee alloplastic reconstruction in patients with advanced gonarthrosis, and in patients who underwent knee arthroscopy the recovery time is shortened. Viscosupplementation is an effective and safe treatment for gonarthrosis. It may be either a complementation or an alternative to other noninvasive techniques.
