Fluoride release from second generation of varnish with CPP – ACP complex under in vitro and in vivo conditions using an ion-selective electrode

Wprowadzenie. Lakiery fluorkowe to preparaty zawierające wysokie stężenia fluoru, stosowane w stomatologii do leczenia i zapobiegania chorobie próchnicowej, często wzbogacane związkami wapnia i fosforu. Skuteczność tych preparatów oceniana jest między innym ilością uwalnianego fluoru w określonym czasie. Cel. Celem badania była ocena dynamiki uwalniania jonów fluorkowych uwalnianych z lakieru MI Varnish firmy GC w warunkach in vitro i in vivo za pomocą jonoselektywnej elektrody ORION model 9609 połączonej z mikrokomputerem pH/jonometrem CPI – 551 Elmetron. Materiały i metody. Materiał do badań in vitro (30 próbek) został przygotowany z usuniętych zębów ludzkich i podzielony na trzy grupy po 10 próbek każda. Po naniesieniu na nie określonej ilości lakieru MI Varnish umieszczono je w szczelnych pojemnikach zawierających sztuczną ślinę o różnym pH (4, 5, 7). Oznaczono skumulowany poziom fluoru (Fskum) po upływie 1, 2, 24, 48 i 168 godzin. W badaniach in vivo udział wzięło 10 ochotników, u których badany lakier został nałożony na powierzchnie żujące, policzkowe i podniebienne wszystkich zębów. Pomiary poziomu fluoru wykonano czterokrotnie, tzn.: przed aplikacją lakieru, po godzinie, po 2 godzinach oraz po 168 godzinach po aplikacji, zbierając za każdym razem 5 ml śliny mieszanej niestymulowanej. Wyniki. W badaniu in vitro uwalnianie fluoru z badanego lakieru różniło się istotnie w każdej godzinie obserwacji i przy każdej kwasowości środowiska (p ˂ 0,001). Najwyższe przyrosty uwalnianego fluoru odnotowano w ciągu pierwszych dwóch godzin obserwacji w każdej badanej grupie, natomiast najwięcej skumulowanego fluoru zostało uwolnione w środowisku o pH = 4. W badaniu in vivo, przyjmując jako poziom wyjściowy fluoru w ślinie 0,158 ± 0,05 ppm F, już po 1 godzinie od aplikacji wzrósł on 12-krotnie do 1,909 ± 0,427 ppm F, po 2 godzinach nieznacznie spadł, przy czym nadal był 9-krotnie wyższy niż przed aplikacją i wynosił 1,468 ± 0,276 ppm F. Po 168 h osiągnął wartość zbliżoną do stanu początkowego równą 0,195 ± 0,08 ppm F. Wnioski. Lakier MI Varnish zarówno w warunkach in vitro, jak i in vivo uwalniał jony fluorkowe przez cały okres badania, przy czym najwięcej fluoru uwalniał w pierwszej godzinie po aplikacji. W badaniu in vitro zarówno pH sztucznej śliny, jak i czas po aplikacji miały istotny wpływ na ilość uwalnianych jonów fluoru. Również interakcja pH i czasu okazała się istotna statystycznie.
Introduction. Fluoride varnishes are preparations containing high concentrations of fluoride, used in dentistry to treat and prevent dental caries, often enriched with calcium and phosphorus compounds. Their effectiveness is assessed by the amount of fluoride released over a certain period of time. Objective. The aim of the study was to evaluate the dynamics of in vitro and in vivo fluoride ions release from the second generation (MI Varnish) fluoride varnish using the ORION model 9609 ion-selective electrode connected to the pH microcomputer/ CPI - 551 Elmetron ionometer. Materials and methods. In vitro study the test material (30 specimens) were prepared from extracted human teeth and divided into three groups of 10 specimens each. Measured amounts of the examined varnish were applied onto specimens and the levels of cumulative fluoride (Fskum) were assessed at the baseline and after 1, 2, 24, 48 and 168 h from the application with the use of an ion-specific electrode. The specimens were immersed into artificial saliva with pH adjusted to 4, 5 and 7. In vivo study the examined varnish was applied to the occlusal, buccal and palatal surfaces of all teeth of 10 volunteers. Levels of fluoride release were assessed in unstimulated saliva four times: at the baseline and after 1, 2 and 168 h from the application. Results. In vitro study, the release of fluoride was significantly different in each hour and at each acidity of the environment (p ˂ 0.001), with the highest amount of fluoride within the first two hours after application in each group and the highest cumulative release of fluoride in acidic environment (pH4). Under in vivo conditions, assuming salivary fluoride level as baseline 0,158 ± 0,05 ppm F, after 1 h following the application, the fluoride level increased 12-fold (1,909 ± 0,427 ppm F), after 2 h it slightly dropped to 9 -fold higher (1,468 ± 0,276 ppm F) and after 168 h substantially dropped to the baseline (0,195 ± 0,08 ppm F). Conclusion. MI Varnish released fluoride ions for 168 hours both in vitro and in vivo conditions, with the highest fluoride release within the first hour after application. Under in vitro conditions the release of fluoride from the varnish was related to the acidity of the immersion medium and the time after application. Also, the interaction of pH and the time was statistically significant.
