Modern jet-powered commercial aircraft operate at altitudes of approximately 40,000 feet (12,192 m). The environmental conditions at these altitudes prevent normal human functioning. The primary hazard is the excessively low air pressure, and consequently, the partial pressure of oxygen in the air, as well as the very low temperature. Insufficient oxygen in the inhaled air causes hypoxia in the human body, which is a generalized oxygen deficiency in the tissues. The brain is the most sensitive organ to oxygen deficiency, as it requires about 20-25% of the body's resting energy demand. The onset and intensity of hypoxia symptoms and compensatory responses depend on the altitude reached, the rate of ascent, the partial pressure of oxygen in the inhaled air, and the duration of exposure. The initial phase of hypoxia is referred to as the time of useful consciousness. This is the period during which operational tasks can be performed effectively and safely, after which mental confusion and loss of consciousness occur. Experimental medical studies indicate that at an altitude of 30,000 feet (9,144 m), the time of useful consciousness is approximately 60-120 seconds, but in the case of explosive decompression, it is halved. In real-world conditions, the time available for analysing and assessing the situation and implementing appropriate emergency procedures may be even shorter. To ensure the environmental conditions required for the proper functioning of the human body at medium and high altitudes, appropriate overpressure and temperature are maintained in the pressurized cabin space of commercial aircraft. The cabin pressure increases with the altitude of the flight and at the operational ceiling, it is approximately 8.30 psi (pound-force per square inch). In the event of a breach in the pressurized space, such a significant pressure difference on both sides of the fuselage causes a rapid and dangerous decompression. Depending on the circumstances and conditions of its occurrence, it can be gradual, rapid, or explosive. This article discusses the various types of decompression and the associated hypoxia hazards, based on real incidents in commercial aviation.

Nowoczesne odrzutowe samoloty pasażerskie operują na wysokościach ok. 40 000 stóp (12 192 m). Warunki środowiskowe na tych wysokościach uniemożliwiają normalne funkcjonowanie ludzkiego organizmu. Główne zagrożenie stanowi zbyt niskie ciśnienie atmosferyczne, a co za tym idzie — niskie ciśnienie parcjalne tlenu w powietrzu oraz bardzo niska temperatura. Niedobór tlenu we wdychanym powietrzu powoduje hipoksję, czyli ogólnoustrojowy niedobór tlenu w tkankach. Mózg jest najbardziej wrażliwym narządem na niedotlenienie, ponieważ zużywa ok. 20–25% zapotrzebowania energetycznego organizmu w stanie spoczynku. Pojawienie się i nasilenie objawów hipoksji oraz reakcje kompensacyjne zależą od osiągniętej wysokości, szybkości wznoszenia, ciśnienia parcjalnego tlenu we wdychanym powietrzu oraz czasu ekspozycji. Początkowa faza hipoksji to tzw. czas użytecznej przytomności – jest to okres, w którym możliwe jest skuteczne i bezpieczne wykonywanie czynności operacyjnych, po którym następuje dezorientacja i utrata przytomności. Badania medyczne wskazują, że na wysokości 30 000 stóp (9 144 m) czas użytecznej przytomności wynosi ok. 60–120 sekund, jednak w przypadku dekompresji eksplozyjnej czas ten skraca się o połowę. W warunkach rzeczywistych czas dostępny na analizę sytuacji i wdrożenie odpowiednich procedur awaryjnych może być jeszcze krótszy. Aby zapewnić odpowiednie warunki środowiskowe niezbędne do prawidłowego funkcjonowania organizmu ludzkiego na średnich i dużych wysokościach, w przestrzeni kabiny pasażerskiej utrzymuje się odpowiednie nadciśnienie i temperaturę. Ciśnienie w kabinie wzrasta wraz z wysokością lotu i na pułapie operacyjnym wynosi ok. 8,30 psi (funtów na cal kwadratowy). W przypadku naruszenia przestrzeni ciśnieniowej tak duża różnica ciśnień po obu stronach kadłuba powoduje szybkie i niebezpieczne rozprężenie. W zależności od okoliczności i warunków jego wystąpienia, może mieć ono charakter stopniowy, szybki lub eksplozyjny. Niniejszy artykuł omawia różne typy dekompresji i związane z nimi zagrożenia hipoksją, opierając się na rzeczywistych incydentach w lotnictwie komercyjnym.
The article presents what decompression is, how to perform it correctly using dive ascent tables, and the typical symptoms of decompression sickness. The article also describes typical diving accidents caused by poorly planned diving without decompression stops being performed. The article emphasises that important factors that contribute to diving safety are: proper training of the diver, a properly prepared diving plan including decompression stops, the diver’s mental state and their so-called adaptive training which helps the diver to manage stress in hyperbaric conditions, preparing their body to undergo correct decompression.
This article is a further one in an unintended series concerning the design of diving technology [1,2,3]. It contains answers to questions raised by readers upon reading previous articles and by users of the systems described therein by way of example. The articles also refer to the discussion held at the annual NATO Working Group meeting on the various types of decompression presented by the Polish side [4,5,6,7]. The previous articles were linked to the acceptance of the results of the project No. DOB-BIO8/О9/О1/2016 carried out under the contract with the NCBiR National Centre for Research and Development entitled “Decompression schedules for MCM/EOD II diving” carried out in 2016-2021. The current article is linked to the new project No. DOB-BIO-12-03-001-2022 implemented under the contract with the NCBiR entitled: “The effects of combat effort and air transport on the safety of combat divers in the execution of underwater combat operations,” scheduled for 2023-2025.
Here we present a unique life path of Sergiy Gulyar, a world-known Ukrainian scientist who was overcoming extreme conditions studying them on himself. He has developed his determination and responsibility from his basic experiences as a surgeon and his desire to win from his involvement in sports. His research in underwater laboratories has shown a capacity to find untrodden pathways to understand and explain what others did not even suspect. His physiological studies on the role of sea depths, breathing mixtures, underwater exposures, and diving schemes marked the path to the aquanauts and undersea man. Reaching the hyper depths equivalent to 2,500 m revealed the physiological limits of human being and defined how to handle hyperbaric respiratory failure. Prof. S. Gulyar suggested a usage of natural physiological mechanisms to accelerate the re-adaptation process as a part of the highmountain rehabilitation of hyperbaria-adapted people. Dissertations were defended, books and articles were written, a scientific school of followers was created. Unfortunately, during the Soviet period, Prof. S. Gulyar faced a set of organizational problems and obstacles from the Soviet regime. In particular, his works were classified or hushed up, his intellectual property was often used without mentioning the author and his scientific team was intimidated by criminal investigations. Main recognition during this period had come from professional societies in Europe and the United States. After the collapse of the Union of Soviet Socialist Republics (USSR), Prof. S. Gulyar introduced a number of innovations and inventions in electromagnetic medicine and physiology. In particular, he has managed to account for main common features of physiological effects of light stimulation produced by Lasers, Light-Emitted Diodes and Bioptron light sources. By doing so, some mystical dogmas were filtered out and new paths to sensible light-induced treatments were developed. At the same time, Prof. S. Gulyar has preserved the legacy of many generations of medical professionals who used light in their treatments. He has shown in his physiological experiments that stimulation of biologically active zones including acupuncture points light stimulation has a wide spectrum of biological effects including alleviation of pain symptoms. Now mono- and polychromatic visual and transcutaneous light therapy of pain has been recognized scientifically and clinically, and its place in medicine has been firmly established. Prof. S. Gulyar described a new functional system of the organism that regulates the electromagnetic equilibrium. A step into the future was the first experimentally grounded technology for the use of fullerene-modified light. Positive changes have been proven with its percutaneous and ocular use. These first results open the door to complete analysis and future investigations. Prof. S. Gulyar has published 20 monographs, 470 papers and abstracts, and received 11 patents. Many of his inventions have been implemented, the others are still awaiting implementation. This article is based on the data obtained by the authors during many years of their personal cooperation, as well as from the memoirs of Prof. S. Gulyar and the materials he provided.

Przedstawiamy wyjątkowe życie Sergieja Gulyara, znanego w kręgach światowych medycyny w tym medycyny podwodnej ukraińskiego naukowca. Pokonał ekstremalne warunki, zdobywając samodzielnie. Jego wykształcenie jako chirurga zaszczepiło w nim determinację i odpowiedzialność, natomiast sport zaszczepił w nim wolę zwycięstwa. W pionierskich badaniach w podwodnych laboratoriach ujawniły jego zdolność do rozwiązywania problemów, by znaleźć i wyjaśnić to, z czego inni nawet nie zdawali sobie sprawy. Jego badania fizjologiczne nad rolą głębin morskich, z zastosowaniem mieszanin oddechowych, metod nurkowania utorowały drogę akwanautom. Doświadczalne badania zjawisk oddechowych na hiper głębokości równej 2500 m rozszerzyło wiedzę na temat fizjologicznego ograniczenia człowieka i określiło sposoby pomocy przy hiperbarycznej niewydolności oddechowej. Prof. S. Gulyar zaproponował wykorzystanie naturalnych mechanizmów fizjologicznych do przyspieszenia procesu readaptacji osób zaadaptowanych do hipierbarii poprzez ich rehabilitację wysokogórską. Zdobywał stopnie naukowe, publikował książki i artykuły oraz założył szkołę naukową, w której znaleźli się jego zwolennicy. Niestety, w okresie sowieckim prof. S. Gular napotykał na szereg problemów organizacyjnych i przeszkód ze strony władz sowieckich. W szczególności jego prace były utajniane lub przemilczane, jego własność intelektualna była często wykorzystywana bez podania autora, a jego zespół badawczy był zastraszany przez dochodzenia kryminalne. Główne uznanie w tym okresie pochodziło od towarzystw zawodowych w Europie i USA. Po upadku ZSRR prof. S. Gulyar dokonał również szeregu innowacji i wynalazków z zakresu medycyny i fizjologii elektromagnetycznej. Jego wyniki wyniki otwierają drzwi do szczegółowych analiz i przyszłych badań. Opublikował 20 monografii, 470 referatów i rozpraw oraz uzyskał 11 patentów. Wiele z jego wynalazków zostało zrealizowanych, inne wciąż czekają na realizację. Artykuł powstał na podstawie danych zebranych przez autorów w wyniku wieloletniej, osobistej współpracy, a także na podstawie wspomnień prof. S. Gulyara i przedstawionych przez niego materiałów. The article presents a unique atypical application of the sonography technique and a methodological description of theintroduction of this technique to research. The Bayesian approach applied to validation of the Doppler method for intravasculardetection of the free gas phase instead of typical statistical inference has been demonstrated in the article. It describes theplace of this method in the diving research work conducted in the Polish Naval Academy without any detailed analysis ofthe results achieved in the studies on decompression supported by ultrasonic detection of the free gas phase in venous vessels.It is a commonly held opinion that Doppler ultrasonic detection of the intravascular free gas phase is not a procedure that canbe particularly useful in decompression research. The main objection is that detection of the free gas phase in venous vesselsis a weak function to predict the presence of the free gas phase in tissues and arterial blood, so this method is not suitablefor assessing the risk of decompression. Only a few countries disagree with this commonly held view and use this method toassess the risk of decompression in decompression studies. France has introduced detection of the free gas phase in venousvessels for diving research and then, together with Canada, improved this method, and developed it to a standard form.Based on the published results of the Canadian research, the technique was evaluated at the Naval Academy using statisticalmethods. The Academy accepted and adopted the results of this research and started to use this method in its own researchon decompression over 25 years ago and continues to use it to great effect.

The safe transition from a higher pressure atmosphere to a lower pressure atmosphere is accomplished by planning the decompression process, typically through changes in pressure and/or composition of the breathing mix in a function of time. However, the decompression process is affected by a much greater number of inherent factors than changes in pressure and composition of the breathing mix. Their values should be kept within certain ranges, however, there are circumstances when it is not possible to maintain control over them. In this situation, they become elements of the residual risk of the decompression process. The safety of decompression should be ensured, inter alia, by analysing the residual risk for each execution of the decompression process.

Bezpieczne przejście z atmosfery o wyższym ciśnieniu do atmosfery o niższym ciśnieniu realizowane jest w oparciu o zaplanowany proces dekompresji, najczęściej na drodze zmian ciśnienia i/lub składu czynnika oddechowego w funkcji czasu. Jednak na proces dekompresji ma wpływ większa liczba inherentnych1 czynnikow, niż tylko zmiany ciśnienia i składu czynnika oddechowego, ktorych wartości powinny być utrzymywane w pewnych zakresach. Jednak zdarza się, że kontrola nad nimi nie może być dotrzymana i pozostają one elementami rezydualnego ryzyka2 procesu dekompresji. Bezpieczeństwo dekompresji powinno być szacowane, między innymi, poprzez analizę tego ryzyka dla każdej realizacji procesu dekompresji.
W obecnie stosowanych rozwiązaniach technicznych, komorę chłodniczą, a w szczególności komorę mroźnię należy traktować jako naczynie ciśnieniowe. Powodem powyższego jest zastosowanie szczelnej jej obudowy oraz nowoczesnych, szczelnych drzwi chłodniczych. Obudowa komory na skutek obniżania i podwyższania temperatury wewnętrznej jest narażana na ciągłe zmiany ciśnienia oddziaływującego bezpośrednio na ściany i strop oraz konstrukcję nośną. Spadek ciśnienia powietrza znajdującego się w komorze chłodniczej w wyniku obniżenia się temperatury nazywamy dekompresją.

Due to the common technical solutions, such as modern hermetic doors and other construction elements, cold chambers should be treated as pressure tight objects. Because of inner temperature variations the construction of a cold room is exposed to constant pressure changes. Pressure drop inside a cold chamber due to inner temperature lowering is called decompression.
The authors discuss the effect of reduced pressure at the final decompression station on nitrogen excretion lrom the organism or a diver. The assumed basi s lor the said considerations was the course ot decompression during dives pertormed in lakes located at a significant altitude above sea level and diver transportation by piane lollowing dive completion. Based on the presented calculations the authors conclude that air transport can take place only upon the lapse ot time calculated with regard to the diving conditions and the expected altitude of the f1ight. Oiving in mountain lakes requires proper consideration of the effects ot the decreased atmospheric pressure.

Autorzy omawiają wpływ obniżonego ciśnienia na stacji końcowej na wydalanie azotu z ustroju płetwonurka. Jako podstawę do rozważań przyjmują przebieg dekompresji podczas nurkowania w jeziorach położonych na znacznej wysokości n.p.m., oraz podczas transportu płetwonurka samolotem po ukończonym nurkowaniu. W oparciu o przedstawione wyliczenia stwierdzają, że transport płetwonurków drogą lotniczą może mieć miejsce dopiero po upływie określonego czasu, uzależnionego od warunków nurkowań, oraz przewidywalnej wysokości lotu. Nurkowania w jeziorach górskich wymagają uwzględnienia wpływu obniżonego ciśnienia atmoslerycznego.
Istnieje potrzeba badania sprzętu działającego na dużych wysokościach i transportowanego drogą powietrzną. Badanie dotyczy również wpływu szybkiej dekompresji. W pracy znajdujemy omówienie wymagań dotyczących badań i propozycje podstawowych parametrów stanowiska badawczego. Bardzo trudne jest zaprojektowanie stanowiska tak, aby czas dekompresji był zagwarantowany. Sprawdzenie tego warunku zostało zrealizowane.

Testing of equipment used at high altitude and transported by air is necessary. The effect of decompression should be tested, too. The requirements for testing and the most important parameters of the stand are presented in the paper. The required time of decompression should be achieved during the tests. The time of decompression in the projected stand is analysed and calculated.
Brak dowodów na zależną od czynnika tkankowego aktywację układu krzepnięcia krwi i uwalnianie trombiny z jednej strony a spadek stężenia czynnika XII po krótkotrwałych ekspozycjach powietrznych oraz saturowanych powietrznych i helioksowych, oraz podwyższone stężenie kompleksu plazminaantyplazmina (PAP) po krótkich nurkowaniach wskazują na możliwość wpływu nurkowania i dekompresji na aktywację fibrynolizy. Celem naszych badań była weryfikacja hipotezy zakładającej, że nurkowanie i dekompresja aktywują układ fibrynolizy oraz wyjaśnienie patomechanizmu tej aktywacji. W badaniach uczestniczyło 50 zdrowych ochotników, których poddano krótkotrwałym, powietrznym ekspozycjom hiperbarycznym pod ciśnieniem 400 kPa i 700 kPa odpowiadającym nurkowaniu na głębokość 30 m i 60 m. Dekompresję stosowano zgodnie z tabelami Marynarki Wojennej. Przed ekspozycją hiperbaryczną oraz po zakończeniu dekompresji we krwi żylnej określano: aktywność czynnika XII, stężenie i aktywność t-PA, stężenie i aktywność PAI-1, stężenie alfa2-antyplazminy, stężenie PAP, stężenie elastazy neutrofili. Stwierdzono istotny statystycznie wzrost aktywności czynnika XII, wzrost stężenia kompleksu PAP, przy jednoczesnym spadku statystycznie istotnym spadku aktywności α2-AP. Nie stwierdzono mierzalnej aktywności t-PA oraz istotnych zmian stężenia t-PA. Stwierdzono istotny statystycznie spadek zarówno aktywności jak i stężenia PAI-1, silniej zaznaczony po ekspozycjach odpowiadających nurkowaniu na 60 m. Stężenia elastazy granulocytow nie różniły się istotnie przed ekspozycją i po dekompresji. Wnioski: U osób kwalifikowanych do nurkowania powinno się sprawdzać czynniki ryzyka zwiększonej aktywności fibrynolitycznej – zaburzenia hemostazy zwiększające ryzyko krwawienia, możliwość występowania zakrzepów/skrzeplin przyściennych.

The lack of evidence for the tissue-factor dependent activation of the coagulation system and the release of thrombin on one hand, and a decreased concentration of factor XII after short term air, saturated air and heliox exposures, as well as an increased concentration of the plasmin-antiplasmin complex (PAP) after short dives indicate that diving and decompression possibly affect fibrinolysis. The aim of our research was to verify the assumption that diving and decompression activate the system of fibrinolysis and the clarification of the pathomechanism of this activation. The study involved 50 healthy volunteers who were subjected to short-term, air hyperbaric exposures at 400 kPa and 700 kPa, which correspond to 30m and 60m dives. Decompression was applied in accordance with Naval tables of decompression. Before hyperbaric exposition and after decompression the following factors were determined: activity of factor XII, concentration and activity of t-PA, concentration and activity of PAI-1, concentration of alpha2antiplasmin, concentration of PAP, concentration of neutrophil elastase. The following observations have been made: a statistically significant increase in the factor XII activity, increase in the PAP complex concentration and a simultaneous significant decline in the α2-AP activity. No measurable t-PA activity or significant changes in t-PA concentration have been observed. In addition, a statistically significant decline in both the activity and concentration of PAI-1 has been observed, which was more pronounced after the expositions that corresponded to 60 m dives. The concentrations of granulocyte elastase did not differ significantly before and after decompression. Conclusions: People qualified for diving should have the following risk factors examined: risk factors of increased fibrynolytic activity – haemostasis abnormalities that increase the risk of haemorrhage, possibility of parietal blood clots/thrombi.
Metoda stanów granicznych (SG) w postaci opisanej w normie Eurokod 2 (PN-EN 1992-1-1, [1]), poza omówioną wcześniej grupą stanów granicznych nośności elementów sprężonych („Nowoczesne Budownictwo Inżynieryjne” 2015, nr 1 [58], s. 64–69), wymaga od projektanta sformułowania, a następnie sprawdzenia warunków stanów granicznych użytkowalności (SGU). Warunki te dotyczą cech użytkowych konstrukcji, jej trwałości, estetyki oraz sprawdzenia zgodności przyjętych założeń obliczeniowych z rzeczywistą pracą elementów.

The limit state method as described in Eurocode 2 (PN-EN 1992-1-1, [1]), in addition to the previously discussed group of ultimate limit states of prestressed elements (NBI 2015, issue 1 [58], pp. 64-69) requires from a designer to formulate and then check the serviceability limit state conditions. These conditions relate to the functional characteristics of the structure, its durability, aesthetics and checks of the compatibility of the design calculation assumptions with actual work of the elements.
The described saturation diving technology was launched as training for Polish Navy specialists. This article is second in a series of articles containing the results of studies conducted in the Naval Academy of Gdynia and financed from educational funds in the years 2009 - 2011 within a developmental research project No. O R00 0001 08 titled: Designing decompression in combat missions. The first article included the results of research into the possibilities of selecting decompression for mix SCR CRABE SCUBA. This paper contains the results of a current research on the possibilities of conducting nitrox-oxygen exposures for oxy CCR/Nx - SCR AMPHORA SCUBA, however it also includes references to the theories that have been published in the first article.
Nurkowanie ma też wpływ na stan czynnościowy śródbłonka naczyń. Brak jest obecnie doniesień dotyczących wpływu nurkowań na ekspresję genów odpowiadających za funkcje śródbłonka. Celem przeprowadzonych pilotażowych badań była ocena wpływu kspozycji hiperbarycznych i dekompresji na ekspresję genów mających znaczenie w biologii komórek śródbłonka. W badaniach wzięło udział 5 zawodowych nurków. Zostali oni poddani krótkotrwałym ekspozycjom hiperbarycznym (0,7 MPa) odpowiadającym nurkowaniom na głębokości 60 m. Dekompresję przeprowadzano zgodnie z tabelami Polskiej Marynarki Wojennej. Ocenę ekspresji genów przeprowadzono metodą RT-PCR z użyciem zestawów RT2 PCR Profiler Array (Superarray Bioscience, a Qiagen Company, USA) oraz cyklera CFX 96 Bio-Rad. Badania ekspresji genów i analiza wyników wykazały zmniejszoną ekspresję genów BCL2 i BCL2A1 oraz ICAM-1 i PECAM-1. Wskazane są dalsze badania w celu ustalenia, czy wykazany w badanej grupie nurków spadek ekspresji łącznie genów PECAM-1, ICAM-1, BCL2, BCL2A1 może mieć udział we wzroście ryzyka wystąpienia patologicznych zmian kostnych po dekompresji.

Diving has some impact on the functional status of the vascular endothelium. There are currently no reports on the effects of diving on the expression of the genes responsible for the functions of the endothelial. The aim of this pilot study was to evaluate the effects of hyperbaric exposure and decompression on the expression of genes relevant to the biology of endothelial cells. The studies were carried out on five professional divers. They were subjected to short-term hyperbaric exposures (0.7 MPa), corresponding to diving at the depth of 60 m. The decompression was carried out in accordance with the tables of the Polish Navy. The evaluation of the gene expression was performed using the RT-PCR method, with sets RT2 PCR Profiler Array (Superarray Bioscience, a Qiagen Company, USA) and CFX cycler Bio-Rad 96. The studies of gene expression and the analysis of results showed a decreased expression of genes BCL2, BCL2A1, ICAM-1 and PECAM-1. Further studies are recommended to determine whether the decrease in the expression of genes including PECAM-1, ICAM-1, BCL2, and BCL2A1 may contribute to the increase in the risk of pathological bone changes after decompression.

Pomimo doniosłości teorii opracowanej przez H.V. Hempelmana jej podstawy teoretyczne nie były szeroko publikowane. W artykule podano wyprowadzenie algebraicznych modeli matematyczny stanowiących podstawę metody Hempelmana służącej do kalkulacji dekompresji.

The paper is the continuation of the review of deterministic and statistical theories of decompression published in the pages of Polish Hyperbaric Research. Despite the importance of the theory developed by H.V. Hempelman, its theoretical foundations have not been widely published. The article describes the steps to derive algebraic mathematical models underlying Hempelman’s method used to calculate decompression.
